Introduction including overview of area inspected and activities taking place

The health and safety inspection was carried out at LARSEN AND TUBRO MODULAR
FABRICATION FACILITY, HYDRO CARBON ENGINEERING at Chennai,India.This,facility is a part
of LARSEN & TURBO headquaters situated at, manapakkam, acknowledged as an out- standing
corporate green campus it is spread over 27 acres, in an eco- friendly surroundings with an ample
greenary almost every concreate structure in the campus has won prestigious awards for excellence
in construction .the campus accommodates 1.4lakhsq.m of built up area with a significant capacity.

1. The report is on one of the major and prestigious project of L&T is FORWARD MOUNTED
BLOCK project which is fabricating the two RIG JACKETS (N1 AND N2). The estimation cost of the
project is 400 crores fabricating in 14 acre site. to meet the IGC3 practical requirement the site was
inspected on 23 JULY 2017 its been inspected in a detailed manner and visually carried out from the
main gate, fabrication site and the mobile shelter area.

2. The machinary and the work equipment which are used in the site are man lift, man basket,
welding machine, grinding machine (AG 4, AG 5, AG 6, AG 7, AG 9), tyre mounted crane, crawler
crane (250 MT and 600 MT), gas cutting equipment, flammable and explosive cylinders, forklift, over-
head cranes (EOT and ganetry cranes).the main work activity are involved in this project are erection
work, fabrication work, grinding, ,gas cutting, welding ,manual handling, lifting activity, man basket
operations, working at height, man lift operations, pressure testing activity, electrical works, blasting
and painting works.

3. Now, the man power details of the project are 1 project manager, 1 safety manager, 3 safety
engineer, 10 safety supervisor, 14 engineers, 3 site in-charge, 24 production supervisors, 3
accountants, 425 workers. with 1 assembly point and 14 fire extinguisher point, and wind direction are
placed on the top corners of the workshop, There is the free access to reach to the assembly point,
welfare facilities such as rest room, first aid room ,drinking water facilities and washing facilities are
arranged within the site.

Executive Summary

I would like to thank to the L&T management, engineers and the supervisors who has supported me
to successfully complete my health and safety inspection .The organization follows some of the good
practices like conducting safety induction to the visitors and the new workers ,and maintaining the
visitors register ,providing mentors to the visitors and emergency contact numbers ,maintaining the
safety notice board at the site and the entrance of the gates.

When the inspection was carried out in the work place I have identified some of the good
practices and simultaneously I have observed some unsafe practices such as:

Flammable and explosive cylinders are stored at the same place without any signage and segregation
which results in the fire and explosion , environmental pollution and severe injuries.

Working on mobile elevated working platform like man lift and man basket are not following the
safety precautions which leads to fatality
The high voltage electrical cable is routed in the pedestrian way and damaged insulated cable are
entangled to the scaffold, results in the electrical shock and property damages

Welding fumes and sparks, cutting sparks ,and simultaneously work activity which leads to the
workers are exposing on the line of fire which causes severe skin burns , ill health effects,
asphyxiation.

So, the following hazards implies the breaches of C 167- safety and health in construction
convection,1988(No.167) Article (26),(27),(29)and (17).These breaches attract the enforcement
actions ,from fines and prosecution from sequent incidents like accidents and injuries.

The management has to fulfill the health and safety obligations to comply with the health and safety
standards to avoid prosecutions and the Breach of law

The unattended loose iron scrap materials ,and tools are left on the scaffold platform and lack of
knowledge on wearing full body harness ,and left the electrodes in the scaffold tubes which leads to
falling of objects and the person causing fractures and fatality.

By showing management commitments towards health and safety by giving appropriate training on
work at height, manual handling, fire and electricity, constant supervision and regular inspections and
so by possibly prevent accidents, legal and financial consequences.

Some of the good practices were also observed which encourage or improve safety culture among
the workers are like the welfare facilities are adequate and HSE policy, and the routes were
segregated for the pedestrians and the vehicles, emergency contact numbers and the first aiders
contact numbers displayed in the site notice board and provided medical center inside the
organization.

It was observed that the workers were using damaged insulation cables and it was entangled to the
scaffold which results in the flow of electricity through the entire scaffold and causes electrical shocks.
found that the grinder was pulling the plug out by tugging at the flex and the people are working near
the connection point which results in causing electrical shocks if the live cable is exposed to the
works, property damage and also found that the high voltage 440volts electrical cable laid down in the
walkway which results in the tripping hazard and property damage and causes severe electric shock
or fatality
The management has to restrict these activities in order to avoid the breach of law and standards

inspections, and providing inspection tags, and conducting trainings and awareness programs on
electrical hazard and activities, By monitoring and providing proper instructions given by the
supervisor and information prior to work.

Observation No. 13, 6, 19: FIRE HAZARDS

It was found that the workers are performing the simultaneous operations and being exposed under
the line of fire which could result in the severe injuries and skin burns. And observed that the gas
cutting sparks are falling on the oxygen cylinder pallet which is using for the cutting which leads to the
fire and explosion and causes the fatality and property damage. And was that the flammable explosive
cylinder has no proper designated storage area which results in the fire and explosion , property
damage, burns and sometimes it leads to the emergency in the site.The management has to control
these type of potential hazards to avoid breach of law

The management has to concentrate on these type of high risk activities by taking the control
measures like planning the work priority and designated area should be allocated for the cylinders and
maintain the distance with fencing and signage to educate the workers.

Observation No .22 , 21, 14 ,26 : HEALTH HAZARDS

It was observed that the welders are exposing to the dangerous fumes which are emitting from their
work which leads to the asphyxiation and lung diseases. And the workers are lifting heavy load
manually and not following the lifting technique or procedure which causes musculoskeletal disorders.
And I have identified that the sand blasting material are spread the entire work place area which
causes eye irritation, asphyxiation, acute effects, and lung diseases, .observed that the workers are
taking food at the work place which causes ill-health effects, The organization has to take
precautionary measures to control the health hazards the regulations for health hazards as per breach
of legislation and standards are:

So, the management of the organization should follow and reduse the manual handling by using

Observation No. 20, 3, 29 : MOBILE ELEVATED WORKING PLATFORM and EQUIPMENTS

It was observed that the tyre mounted crane out riggers wooden pads which results in the unequal
balance on the out riggers causes crane topple, property damages and fatality.it was observed that
the man lift operator was moving the vehicle from one place to another by raising the cradle which
leads to cradle collide with the adjacent structure, boom collapse and people fall from cradle causing
property damage and fatality. The workers were working in the man basket were over reaching from
the basket and stood on the guard rails on the basket leads to fall from basket causing fatality. The
management has to avoid these types of unsafe activities to comply with the health and safety
legislations and standards.

The organization has to take control measures to avoid accidents/incidents with high risk activity and
mobile elevated working platform and equipments like carrying out of suitable risk assessment, safe
system of work, and procedures to provide adequate and related training and information, instruction
and good supervision to the workers to avoid the legal prosecutions and fines in the court.

observation No. 1, 9, 15, 18 : FALLING HAZARDS

It has been observed that the worker left the electrodes in the scaffold tube which leads to the fall
down of elecrodes when dismantle the scaffold which results in penetrating the electrodes into the
body who works under the area which causes severe injuries and fatality. And observed that the
workers are not maintaining the 3 point contact while climbing the ladder which results to falling
objectives and fall from ladder causes fractures ,severe injuries and also it may lead to fatality which
depends on the height. the workers left the iron scrap materials and tools on the scaffold at the work
place because it results in tripping and falling objects and the person causes head and shoulder injury
and fatality its noticed that the workers does not have the knowledge on the full body harness like the
worker had tied the knot to the lanyard which leads to the improper functioning of the body harness
and when they cause severe injury to the spinal cord and ger injury .

ACCORDING TO:

The management should control these unsafe activites in the work side by providing proper
equipment. training to the workers at work at height. and carry out the risk assessment, and good
supervision.

If there is an accident/incident are occurred regarding the work at height the organisation has to face
the external agencies, enforcement Authorities and legal prosecutions in the courts which results in
direct cost like compensations to the victim, fines and the indirect cost like loss of time and production
and cost of investigation.

Conclusions

The management of L&T are maintaining the good standards of health and safety and the
management allocating the resource to fulfil the health and safety obligations and it follows the good
process like developing the safe system of work, method of statement, carrying out the risk
assessments, and involving the workers while carrying out the risk assessment and maintaining the
permit to work system and improving the health and safety culture by implementing the safety tours,
sudden safety work place inspection by top management, safety awareness programs and safety
award functions.

The health and safety report explains the work place hazards and the consequences are associated
with the hazard and suggest the control measures to prevent the accident/incident at the work place .
increase the high standard of safety by finding and by rectifing the faults in the system and it also
avoids legal prosecution in courts, breach of law and loss occur to the organisation due to accidents
and incidents

The critical observation was found that during performing high risk activity are close materials are on
the scaffold, electrodes inside the scaffold pipe, no proper storage of flammable and explosive
cylinders, and the no proper instructions are not given to the man lift operator and the workers who
are working in the man basket, no proper routing for the high voltage electrical cable, has no clear in
the time scale and needs to train the workers especially who deals with high risk activites like working
at height, hot work and lifting activity. so, that the organisation has to concentrate more on the work at
height activites, which advices the management to concentrate on their health and safety practises,
standards and to develop the safety culture.

To overcome the above unsafe acts and condition that the organisation has to provide with adequate
training to the workers and the staff and proper information and instruction prior to work and continues
monitoring and supervision has to be provided to the workers. Through training, instruction,
information and the supervision that the organisation can avoid the rate of accident and incidents and
injuries at the work place.
