RAFAEL F. TAN, Filipino Citizens, of legal age, and residents of 223 Long
Drive, San Antonio Village, Muntinlupa City, after having been duly sworn in
accordance with law, do hereby depose and state that:

1. We are the petitioners in the above-captioned case;

2. We have caused the preparation of the foregoing Petition for Review on Certiorari;
3. We have read all the allegations therein and base on our personal knowledge and
official records and documents at hand, these allegations are true and correct;
4. We certify that we have not commenced any action or proceeding involving the
same issue/s in the Petition for Review on Certiorari in the Supreme Court, the Court
of Appeals, or different Divisions thereof, or any other tribunal or quasi-judicial
agency;
5. To the best of our knowledge, no such action or proceeding is pending in the
Supreme Court, the Court of Appeals, or different Divisions thereof, or any
other tribunal or agency; and,
6. If we should hereafter learn that a similar action or proceeding has been filed or
is pending before the Supreme Court, the Court of Appeals or different
Divisions thereof, or any other tribunal or agency, we undertake to promptly
inform this Honorable Court of this fact within five (5) days from our knowledge
thereof.
