Guide to completing placement hours log

To record your hours of practice as a student nurse, please fill in the details for each of your 8 placement experiences. You are required to experience a total of 2500 hours
at the end of your 3 years of training, so adding up your placement hours should reflect this. For each placement experience you should record the key learning points
obtained from this area.

Guide to completing feedback log

You will need to provide details of feedback you have received whilst on placement. Each year of your training, you will need to record the details of a required amount of
feedback, which will add up to 5 pieces over the 3 years:

 Year 1 – 1 x piece of feedback


 Year 2 – 2 x pieces of feedback
 Year 3 – 2 x pieces of feedback

You should not record any information that might identify an individual, whether that individual is alive or deceased. You might want to think about how your feedback
relates to the NMC code, and how it can be used in your reflective accounts.

Reflective account:

What was the nature of the practice-related feedback and/or event or experience on your placement?
Feedback was given by a member of the nursing team based on a shift shadowing them in a bay on a hospital
ward. I was expected to perform the routine tasks of nursing in such a context, including recording of findings,
patient repositioning, assisting with ADL’s, meeting nutritional requirements and administration of medications for
6 patients for a full shift under supervision of an RN.

What did you learn from this feedback and/or experience on your placement?
I learned that I have developed substantially in my abilities in the performance of daily routine nursing tasks since I
commenced, which had been a concern of mine as I had no previous experience of ward work. It was reassuring to
learn that I was able to perform those tasks in an acceptable fashion and that there were no concerns about my
ability to work in a safe, patient-centred fashion. While the feedback was positive, I felt it was basic and that I was
capable of performing to a higher degree and making more of an impression.

How did you change your practice as a result?


Whilst the feedback received was generally positive, I felt it could have been better. As such I strove to improve my
focus on patient care in particular, ensuring that their needs were tended to as best as possible, rather than trying
to do everything to an acceptable standard.

How is this relevant to the code?


My effort to prioritise patient wellbeing is directly referenced in the NMC code, “Make sure that people’s physical,
social and psychological needs are assessed and responded to” and “Act in the best interests of people at all times”
being the relevant sections.

You will meet with your personal tutor each year for a face to face reflective discussion on your progress through the
programme.

This will allow you to identify and discuss your strengths and weaknesses, so areas for development can be identified
and actions to achieve these can be planned. It will allow you to reflect on your academic progress and placement
experiences. Lastly, your personal tutor will be able to consider your well-being and assist with any support you may
need with personal matters.

Summary of discussion:
Our tutor group had a meeting as a whole with our personal tutor, Sanj Nathoo, on the 12th of December. We
discussed how to fill in the portfolio, along with the support that is in place for us during placement, both from the
university as an institution and Sanj as our personal tutor.

I feel fairly confident in my progress thus far, although progressing to placement has been a source of nervousness
as hospital-based work is outside my experience and I feel expectations will be high.

Otherwise I am just looking forward to completing year one and proceeding to year two.
