I. Rationale:

AMENDING PROCLAMATION NO. 1895, BY AUTHORIZING THE SCIENCE


PROMOTION INSTITUTE OF THE NATIONAL SCIENCE AND TECHNOLOGY
AUTHORITY AND THE MINISTRY OF EDUCATION, CULTURE AND SPORTS IN
COORDINATION WITH THE PHILIPPINE SOCIETY OF YOUTH SCIENCE
CLUBS, INC., THE SCIENCE CLUB ADVISERS ASSOCIATION OF THE
PHILIPPINES, INC., AND THE PHILIPPINE ASSOCIATION OF SCIENCE AND
MATH SUPERVISORS, INC., TO IMPLEMENT PLANS AND ACTIVITIES FOR
THE CELEBRATION OF SEPTEMBER OF EVERY YEAR AS NATIONAL
SCIENCE CLUB MONTH.
WHEREAS, Proclamation No. 1895 was issued on 4 September 1979 declaring
September as National Science Club Month and authorizing the former Science
Foundation of the Philippines in coordination with the Philippine Society of Youth
Science Clubs, Inc., and the Science Club Advisers Association of the Philippines to
implement plans and activities for the same;
WHEREAS, pursuant to Executive Order No. 784, the Science Foundation of the
Philippines was abolished and in its place was created the Science Promotion Institute, an
organic agency of the National Science and Technology Authority which is charged with
the responsibility of promoting public consciousness on science and technology
throughout the country;
WHEREAS, it is necessary and appropriate to recognize the Ministry of Education,
Culture and Sports and the Philippine Association of Science and Math Supervisors, Inc.,
as active partners in the planning and implementation of activities pertaining to the
Proclamation;
WHEREAS, the youth, considered as valuable human resource has been identified as the
most instrumental population group for the promotion of science consciousness, public
understanding and efficient utilization of the results of research and development on
science and technology to the country particularly at the grassroots levels;
WHEREAS, the Science Clubs and Societies could be the most effective vehicles in
developing a science-conscious citizenry through their various experiential and
participative activities for the youth which help instill in the Filipino a deep commitment
to science and technology as a pre-condition to economic development and growth;
In this activity, it aims to teach, inspire and make students learn something that they can
use and apply in their lives. the organization can earn funds that can be used for the
whole academic year.

II. Description:

Objectives
This Math and Science week program entitled “ ” aims to achieve the following:
a) To inspire the students and eventually appreciate the essence of mathematics
b) To make the students happy and enjoy the math exhibit
c) To give additional knowledge to students in terms of trivia’s in mathematics
d) To gain additional fund that can be used by the organization for the whole year.

Number of beneficiaries
All students of Mabalacat City College are the chosen benefactor of the activity.

Duties and Responsibilities:
1. Chair overall committee discussions.
2. Act as a signatory for the committee in all legal purposes and financial purposes.
3. Serve as a spokesperson when required.

Committee on Secretariat: Mariel S. Mallari


Duties and Responsibilities:
1. Prepares the flow of the program.
2. Maintain records of the committee and ensure effective management records.
3. Is responsible for ensuring that accurate and sufficient documentation exists to
meet legal requirements.

Committee on Finance: Romela V. Delos Reyes


Robertson C. Manahan
Duties and Responsibilities:

1. Provide advice to the committee in their management regarding finances.


2. Administer all financial affairs of the La gaMath.

1. Oversees the procurement and distribution of all supplies needed for each work.
2. Assesses the supply needs for the event and makes sure that all committees have
what they need to complete their assignments.
3. Take charge of the documentation for the submission of Accomplishment
Reports.
4. Take charge of the evaluation of the activity after it was accomplished.

III. Project Methodology:

Methods/Strategies used in the Implementation of the Project.


a) Prepare the course of action for the tutorial program with officers and advisers.
b) Collection of the name and fee of students that will undergo the tutorial program.
c) Organize a prayer and Ice breaker before the program starts.
d) Introduce different techniques in solving math related problems that can help the
students.
e) Prepare a short test after the tutorial.

IV. Monitoring and Evaluation Scheme:

On the first week of class, the current members of La gaMath will visit the first-
year student of BSEd Mathematics to introduce themselves, La gaMath Organization
and the said tutorial program. On the same week the organization will promote the
program by giving flyers or by inviting the students.
In every session of the said activity, the organization will monitor the attendance
of the students so that the organization can follow through the students who attended
in a session but did not attend the succeeding session.
A feedback from the students who participated will be collected by the
organization through a survey and document this feedback to serve as a basis for future
purposes. The effectiveness of the activity will be recorded by the organization through
the improvement on their math grades or improvement in their self-confidence.
