Dear Corpuz:

This office acknowledges your INITIAL/RENEWAL/VARIATION application with an


Account Code: 3000002260818  for License to Operate (LTO) as  Drugstore.

However, the application cannot be processed since your establishment has failed to submit the
complete and correct documentary requirements as listed below:

Pursuant to Section 31 (a) (1) Article IV of the Republic Act No. 10918
– Pharmacist Requirement of the Philippine Pharmacy Act states that

2. the LTO has already expired at the time of receipt of the disapproval letter but re-application
was made within thirty (30) calendar days;
3. complete requirements were submitted; and
4. corresponding fees were paid

Application filed contrary to the above conditions shall be automatically cancelled and delisted
