PURPOSE
The purpose of this plan is to establish parameters that for the handling and investigation of
accidents, incidents and non-conformances by taking action to mitigate any consequences
arising from accidents, incidents or non-conformances, the initiation and completion of
corrective and preventive actions and confirmation of the effectiveness of corrective and
preventive actions taken during all Electro Fibre projects.
SCOPE
This procedure applies to the following:
2.1 All Occupational Health and Safety related incidents, accidents, non-conformances and
near misses that cause injury/illness or have the potential to cause injury/illness to
employees during all work on site.
2.2 All damage to services and property by employees of Electro Fibre while performing
their duties.
RESPONSE TO INCIDENTS
Disabling incidents
All serious and major incidents shall be reported as required by Section 24 and GMR 7 of the
Occupational Health and Safety Act. The H&S Manager shall be notified immediately and a full
investigation shall be undertaken as detailed in the Occupational Health and Safety Act

The CR 8(7) or CR 8(8) must report all near miss incidents to the Line Manager / Supervisor and

Incident Investigator for record purposes.
All High Potential incidents need to be reported to the Incident Investigator for further investigation.

These incidents are to be investigated immediately to prevent future injures.
3 All incidents, regardless of the severity, shall be reported to CLIENT .
All first aid incidents must be reported to the nearest First Aider. Supervisor to ensure that suitably

trained first aiders are available. Names of the First Aiders will be displayed and kept up to date.

Service affecting Incidents:
Service affecting incidents occur when employees of Electro Fibre cause damage to the equipment /
cables of other service providers, e.g. Telkom, DFA, and Municipality. These incidents happen
accidently but they could be fatal.
The CR 8(7) must contact the appropriate department immediately. The local municipality must be

contacted if there is a water or electrical cable damage.
If an electrical cable is damaged any work in the vicinity of the damaged cable must be stopped and
workers to evacuate from that area, Supervisor to call the electricity department and report the
damaged cable and obtain a reference number for the report of the damaged cable.

Work will only resume once cable is repaired or if the electricity department confirms that the cable is
completely dead and they furnish the supervisor with a reference number stating that the cable is
either repaired or completely dead/ and or has no power.
3 Ensure the injured person is safe and cordon off the area.
4 The incident must be reported to line manager / supervisor thereafter.
They are to report the incident to the incident investigator. He must investigate the incident and fill in

the require documents.
6 In his report he must note the depth of the cable and the position.
7 Report incident to safety officer and ILO and PM immediately as per Incident report form.

Damage to Property
Damage to property could be trench in a garden or across a driveway. It could also be damage to the paint
work on a wall, a fence collapses while trenching or damage to a motor vehicle.
1 CR 8(7) must inform Sect 16(2) Appointee of damage.
2 Supervisor to contact the property owner immediately.
Incident investigator must investigate the incident and must take measures so that this incident is not

repeated.
4 Site Supervisor must report incident to m safety officer, ILO or PM.
5 3 Core to ensure that the damages are repaired timeously.
6 Seek owner’s permission prior to trenching in his garden or across his driveway
