AFFIDAVIT OF COMPLAINT

The UNDERSIGNED, MARIO TORRES, accuses JUAN MIGUEL

and JUN MIGUEL, of slight physical injuries, committed as follows, to
wit:

(a) That about 12 o’clock midnight of February 5, 2019, in Zarate

Village, Tacay Road, Barangay Pinsao Proper, Baguio City,
Philippines, the said accused under the influence of alcohol
threw bottles of beer that hit the back-left leg of the complainant
causing superficial wound;

(b) That instead of helping the victim, accused Juan and Jun escaped
towards their house located near a stone crusher, same
barangay.

(c) That during the event Aling Nena Cruz witnessed it since she
was the store owner where the incident happened. Attached
Statement of Witness in Annex “A”;

(d)That Mario accompanied by his uncle Ramon Torres went to

Baguio General Hospital for first aid. The wound healed within
nine days. Attached medical certificate in Annex “B”.

(e) That after administering first aid, complainant together with his
Uncle went to the police station to report the incident. Attached
Police blotter report in Annex “C”.

(f) That on February 11, 2019, complainant filed a complaint before

the Barangay and after several hearings, no amicable settlement
was reached between the parties as evidence the Certificate to
File Action is attached in Annex “D”.
 The undersigned executed this affidavit to attest the truthfulness
of the foregoing facts and to support the filing of Criminal Cases
against JUAN MIGUEL and JUN MIGUEL for Slight Physical Injuries
in violation of Art. 266 of the Revised Penal Code.
