AFFIDAVIT OF DISCREPANCY IN NAME

I, Eduardo Soriano Solis, of legal age, single, Filipino, and with address at 1015 Pandacan Street
Pandacan, Manila, under oath, state:

1. THAT I was born on May 25, 1960 in Sta. Ana, Manila to the spouses Edmundo Pablo Solis
and Fe Soriano Solis;

2. THAT I was baptized on July 8, 1960 at Sto. Nino Parish in Pandacan, Manila with the name
Eduardo Soriano Solis as appearing on my Baptismal Certificate. A copy of my Baptismal
Certificate is hereto attached as ANNEX “A”;

3. THAT since childhood, I am known by the name Eduardo Soriano Solis which I have been
using in all my personal records, transactions and communications. A copy of my Transcript
of Record, Driver’s License and Passport are hereto attached as ANNEXES “B”, “C”, and
“D”, respectively;

4. THAT the name appearing on my Certificate of Live Birth registered with the Civil Registry of
Manila and with the National Statistics Office is Edgardo Soriano Solis. A copy of my
Certificate of Live Birth issued by the Civil Registry of Manila and the National Statistics Office
are hereto attached as ANNEXES “E” and “F”, respectively;

5. THAT the names Eduardo S. Solis and Edgardo S. Solis refer to one and the same person,
the herein Affiant;

6. THAT since it will be easier to correct one document which is the Certificate of my Live Birth,
I am executing this two-page affidavit (including the jurat) to rectify my name on said
document and change the first name “Edgardo” to “Eduardo”, the name appearing on my
Baptismal Certificate;

7. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and to use the
same for whatever legal purpose it may serve.
