AFFIDAVIT OF NON-BAPTISM

I, ROBERT KEVEI L. CRUZ, of legal age, Filipino, single, and resident of

7573 Swimming Pool St. Maligaya Park Subd., Camarin Caloocan City, after
having been sworn in accordance with the law hereby depose and state that;

1. I am the uncle of MARIA FATIMA CRUZ, 9 years of age, daughter of my

sister LOUISE JEAN L. CRUZ and residing at 7573 Swimming Pool St.
Maligaya Park Subd. Camarin , Caloocan City;

2. That personally known the child since birth on December 24, 2010 at Blk.
6 Lot 36 Rowena Lane, Diamond Village, Bgy. Kaligayahan, Novaliches,
Quezon City and living with our family since then ;

3. That as of this date, we have not celebrated her baptism as a Roman

Catholic because of her non registration of birth at the local civil registry
of Quezon City.

4. I am executing this affidavit to attest to the truthfulness of the foregoing

statements and for all legal intents and purposes it may serve.
