Consent Form

I Faiqa Sabir (student of M.Sc- III), am going to conduct my research under the

supervision of Ms. Nousheen Ramzan from the Institute of Applied psychology,

University of the Punjab, Lahore. This research has been designed to assess that how

Resilience, Self-compassion and Mindfulness contributes towards the Emotional

Well-being of doctors working in critical care and non-critical care units of the

patients. In this research you will be required to fill the Bio-graphical Form and will

answer the questionnaires related to topic stated above. It will take approximately

thirty minutes to complete the questionnaire. Do not hesitate in answering all the

questions. There is no right or wrong answers.

Your participation is voluntary and you can refuse further participation anytime

you like during research activity. Moreover, this exercise does not involve any risk or

penalty. Your provided information will be kept confidential and will be used only for

research purpose. In the end, if you have any query, please do not hesitate to ask. If

you agree to participate, please sign the consent form.
