AFFIDAVIT

I, GUADALUPE B. TANZO. of legal age, married, a Filipino citizen,

residing at Balila, Lantapan, Bukidnon, after having been duly sworn to in
accordance with law, depose and state:

1. That I am the owner of a Suzuki Utility Vehicle with OR No.

1134595291 and CR No. 172045576 issued on April 6, 2017, and
Plate No. 102004;

2. That the said Utility Vehicle has a for hire boundary of Kibawe to
Kisolon and vice versa and has been in service using that same
boundary since;

3. That for personal reasons, the said Utility Vehicle will be used to
travel outside of its usual boundary on April 26 to 29, 2018 to
Ozamis City, Philippines;

4. That I am executing this affidavit for the purpose of applying for a

Petition to Travel Out of Line;

5. That I executed this affidavit voluntarily, read and understood the

contents thereof, especially that the contents thereof were
translated to me in Visayan language, the language I can very well
understand and that the same are all true and correct based on
my personal knowledge and based on authentic documents.
