Department of Environment and Natural Resources Region 4-A in Los Banos Laguna, the technical
descriptions, reads:

The true and correct technical description is shown by the copy of approved lot plan and lot data
computation secured from DENR- Region 4-A in Los Banos, Laguna and not as appearing in the technical
descriptions of the Transfer certificate of Title No. T-123456.

That we executed this affidavit to attest the above-stated facts for all legal intents and purposes.
