Lidocaine, an aminoethylamide derivative of xylidine, was the first amide local anesthetic
suitable for nerve block injection in dentistry. It is approximately two times as potent and as toxic
as procaine. When injected intraorally, lidocaine produces a greater depth of anesthesia with a
longer duration and affecting a larger area, than an equivalent amount of procaine. As a result,
lidocaine became the most popular local anesthetic agent available in a dental cartridge, and it is
considered the prototypic amide agent. Lidocaine hydrochloride is available under a variety of
trade names ; several manufacturers also market the drug by its nonproprietary (generic)
designation .
vasoconstrictor or with I : 100,000 (10 gg/mL) or 1:50,000 (20 ug/mL) epinephrine (or its
hydrochloride or bitartrate equivalent). The original formulation (Xylocaine, Astra) also contains
6 mg/mL sodium chloride and, in vasoconstrictor-containing preparations, 0.2 mg/mL citric acid
and 0,5 mg/ml sodium metabolism. Some manufacturers use slightly different amounts of sodium
chloride and other antioxidants, such as not more than 0.55 mg/mL sodium bisulfite or 1.2 mg/mL
potassium metabisulfite with 0.25 mg/mL disodium edetate. Lidocaine is also available outside
the United States with 1:80.000 (12.5 g/mL) epinephrine or with 1:25,000 (40 ug/mL)
norepinephrine. Addition ally, a recent European formulation of 2% lidocaine with both 1:50,000
epinephrine and 1:50,000 norepinephrine has been developed for "maxi mum anesthetic power."
A 3 % lidocaine with 1:25,000 norepinephrine preparation has also been marketed for the same
purpose. Although these latter twe preparations may improve pulpal anesthesia in selected patients,
they substantially increase the risk of toxic reactions. Lidocaine is probably the most versatile local
anesthetic yet discov ered. It has bcen injected by virtually every route available and has proved
highly effective as a topical anesthetic . The most commonly used formulation, 2% lidocaine with
1: 100,000 epineph- rine (or 1:80,000 in countries such as Great Britain, where it is known as
lignocaine, and Japan), provides profound local anesthesia with acceptably low systemic toxicity
and minimal allergic potential. Pulpal anesthesia occurs quickly and lasts sufficiently for most
dental procedures to be completed without the need for reinjection.5 Because of its potent vaso-
dilatory property and inadequate duration of pulpal anesthesia, the plain solution is infrequently
used in dentistry. The 1:50,000 strength of epinephrine, which provides concentration while
increasing the risk of adverse cardiovascular re- sponses, is best reserved for situations where
specific benefit may accrue from its greater vasoconstrictor potential. Indeed, some7.28 but not
all29 N0 investigators have argued that a more dilute, 1:200,000 (5 ug/mL) concentration of
epinephrine is sufficient for most purposes and should be adopted as the standard. In part because
of its unique role in the management of ventricular dysrhythmias, more is known about the
systemic actions and metabolic fate of lidocaine than of any other conventional local anesthetic.
Indeed much of the information in Chapter 2 is based on studies involving lidocaine. In contrast
to other local anesthetics, lidocaine tends to produce a degree of sedation in subconvulsive doses,
and even seizures caused by overdosage are often muted in magnitude compared to procaine.
Convul sions typically occur in blood concentration above 10 ug/mL; however, they may develop
submucosal injection are reached in about 15 minutes; the presence of epinephrine can delay the
attainment of peak values by an additional 5 to 15 minutes. For 2% lidocaine, dose limits approved
with 1:100,000 epi- nephrine at any one time. While the 25-mL dosage limit also applies for 2%
lidocaine with 1:50,000 epinephrine, this formulation is best used sparingly to avoid excessive
administration of vasoconstrictor.
PRILOCAINE HYDROCHLORIDE
The drug is somewhat less potent than lidocaine as an anesthetic and considerably less toxic after
injection into peripheral tissues. As with mepivacaine, prilocaine produces less vasodilation than
does lidocaine and can therefore be reliably used as a plain solution for short procedures.
Prilocaine is similar to other amide local anesthetics in its relative freedom from allergic reactions
(as the bitartrate). Sodium metabisulfite (0.5 mg / mL) and citric acid (0.2 mg / mL) are included
Prilocaine with epinephrine (Citanest Forte) provides anesthesia com- parable in onset and efficacy
to that achieved with 2% lidocaine with 1: 100,000 epinephrine. Pulpal anesthesia may be
somewhat shorter in duration but is sufficient for most operative procedures. Removal of the
vasoconstrictor causes no loss in activity after inferior alveolar nerve block. Anesthesia of the
shorter, with pulpal anesthesia lasting for as little as 10 minutes in some patients.2 As with 3%
mepivacaine, failure of anesthesia may occur because of the rela- tively evanescent action in
maxillary teeth. The advantages to having no vasoconstrictor also apply Prilocaine is the least toxic
amide anesthetic in current use, which permits its safe formulation as a 4% solution. It enjoys a
significantly larger volume of distribution and greater clearance rate than xylidinebased anesthetics
. Because the drug leaves the circula- tion rapidly, peak blood concentrations occur within 15
minutes after intraoral injection and are about half those achieved by similar doses of lidocaine or
mepivacaine. The metabolism of prilocaine is outlined in Although the liver plays a major role in
the biotransformation of prilocaine, clearance of prilocaine from the blood is significantly faster
than its delivery to the liver via the bloodstream. Therefore, the breakdown of the drugs in other
sites is predicted, with the kidneys and lungs being the most likely organs involved. Perhaps
because it is a second ary amine, hydrolysis of the amide bond is the major pathway for metab-
alism of the parent compound. Only a small percentage (<20%) of prillocaine has been recovered
in the urine as the parent drug and its known metabolites The primary factor limiting the clinical
