COMPLAINT-AFFIDAVIT

I, CARDO A. DALISAY JR., minor, Filipino citizen, single, with assistance

of counsel, and resident of 699 Cabildo St, Intramuros, Manila after having duly
sworn to in accordance with law do hereby state under oath that:

1. I am the complainant in the above-entitled case;

2. I accuse and hereby charge respondents BOBBY A. SALAZAR, ALEX B.

CRUZ and TEODORO C. AGPALO all assigned police officers stationed
at Intramuros Police Station for the crime of Slight Physical Injuries
penalized under Article 266 of the Revised Penal Code in relation to
Republic Act 7610 ( "Special Protection of Children Against Abuse,
Exploitation and Discrimination Act.") , committed as follows, to wit:

a) That on September 16, 2019 at around 11 o’clock at night,

while I was a few meters away from my house I encountered
police officers BOBBY A. SALAZAR, ALEX B. CRUZ and
TEODORO C. AGPALO who told me to come with them to
the police station for violating the curfew.
b) That upon arrival at the police station I was immediately
ordered by ALEX B. CRUZ to stand in the corner when
BOBBY A. SALAZAR suddenly punched me in the face
which made me fall on the floor.
c) That when I was laying on the floor, the three police officers
took turns in kicking my stomach and legs. After the three
mentioned police officers beat me up, they carried me
outside the police station and dropped me in the exact place
where I encountered them.
d) That while I was laying there, I, with all my might,
repeatedly shouted “Tulong!” which resulted to my
neighbors to get out of their house and alert my father
CARLO F. DALISAY SR. of my current situation.
e) That on that night I was immediately rushed to the hospital
by my father to have an examination of my swollen face and
 the other injuries that I sustained. Attached here is the
medical certificate issued by the doctor which states that my
injuries would take a period of six days for them to heal.
f) That also, because of the injuries I sustained I was not able to
attend school for 5 days.

3. That I am executing this affidavit to attest to the truth of the foregoing

statements, and for the purpose of charging BOBBY A. SALAZAR, ALEX
B. CRUZ and TEODORO C. AGPALO for the crime of Slight Physical
Injuries penalized under Article 266 of the Revised Penal Code in relation
to Republic Act 7610 ( "Special Protection of Children Against Abuse,
Exploitation and Discrimination Act.").

I, JUAN A. DELA CRUZ, of legal age, Filipino and a resident of 670

Sulucan St. Samplaloc, Manila after having been sworn in accordance with law,
depose and state:

1. That I, as the owner of JDC Corporation, hired CRISTIANA D. SAN JOSE

as my secretary.
2. That as my secretary she is tasked to schedule my meetings, keep certain
files, and answer calls.
3. That her employment has been terminated because upon investigation, it
was found that she was deliberately influencing other people to transfer to
another company and leaking out information to our rival companies
thereby breaching the trust and losing the confidence given to her by ABC
Corporation.
