9O5O Overview
The BACTEC@9050 instrument is designedfor the rapid detection of bacteria
and fungi in clinicalcultures of blood. Samplesare drawn from patientsand in-
jected directly into BACTEC culture vials.Vials
are then entered into the instru-
ment as soon as possibleto insure performanceefficacy.

When microorganismsare present,theymetabolizenutrients in the culture me-


dium,releasingcarbon dioxide into the medium.A dye in the sensor reactswith
COr.This modulatesthe amount of light that is absorbedby a fluorescentmate-
rial in the sensor.Theinstrument'sphoto detectors measurethe levelof fluores-
cence,which correspondsto the amount of CO, releasedby organisms.Then
the measurementis interpreted by the system accordingto preprogrammed
positivity parameters.(SeeFigure l.)

At system start-up,the BACTEC@9050 instrument pedorms self-diagnostics


and loadsits operatinginstructions.Thenthe instrument beginsautomatedtest-
ing.A row of Light EmittingDiodes (LEDs)behindthe vialsilluminate,activating
the vials'fluorescentsensors.Theinstrument'sphoto detectors then take the
readings.Atest cycle is completedeveryten minutes.Positiveculturesare imme-
diately flaggedby an indicator light on the front of the instrument,an optional
audiblealarm,and are displayedon the LCD screen.

When positivevials are identified,the lab technologistpulls them from the in-
strument for confirmation of results.and for isolationand identificationof the
organism.

An instrument is capableof monitoring a total of 50 BACTEC@ culture vials.The


practical capacityis 5 culture sets per day with a 5-daytest protocol.The vials are
arrangedin 3 concentric ringsdesignatedA, B,and C.The vialsare continuously
incubatedat 35' C, and are agitatedfor maximum recovery of organisms.

The BACTEC@9050 instrument (hardware)components are describedin the
followingparagraphsand are shown in Figure2. Controls and indicatorsfor these
\/ modulesare discussedin Section3 - Controls and lndicators.

The malor subsystemsof the instrument includethe following:

Rot'or

The rotor contains50 wells called"vial stations,"into which vialsare placedfor


incubationand testing.Therotor containsthree rings of vial stationsdesignated
A, B,and C. Eachstation is numbereduniquely( | - 50), but the systemprovides
the ring letter designation(A, B,or C) to help you quicklyidentif where to place
or removevials.One row of stations(one station in each ring) containsthe test
LEDs that activatevial sensorsand the photo detectors which take the actual
readings.

The rotor is mounted at a 20o angleand rotates to provide agitationto culture


vials.Agitationof cultures can improve both time to detection and recovery of
organisms.The rotort drive motor stops agitation when the cabinet door is
oDened.

The temperature insidethe instrument is preset to maintainthe internal tem-


\/ peratureat 35'C + 1.5'C.Over- and under-temperature alarmsetpointsare
preset at 35.5" C and 34.5" C, respectively.

The LCD Displayis a240X64 pixel LiquidCrystal Displaythat presentsinforma-


tion about systemstatusand function key definitionsthat enableyou to perform
systemoperations.Key definitionsand statusinformationare identifiedby picture
iconsthat representthe type of informationor operationthat can be per-formed.
A comprehensivelist of systemicons is presentedin Section5 - Reference.

The keypadenablesyou to perform operarionssuch as entering and removing


vials,adjustingsetupparameters,etc.Four of the keys,markedwith printed icons,
per{orm fixed functionsregardlessof the current displayor operation.Six other
keys,teal in color, are software (soft) keyswhose functionsvary dependingon
the current activedisplay.Eachdisplayshows icons representingthe current Soft
key assignmentsat the bottom of the screen.To perform the function repre-
sentedby the icon,you pressthe correspondingSoft key below the icon.

Barcode Scanner

A barcode scanner is located on the front of the instrument to provide the


abilityto scanvial labelsfor specimenidentification.Thescannerturns on auto-
maticallywhenevera barcode needsto be scanned.

Computer
The system computer stores all the system software, includingthe software
which controls instrumentoperationsand the user interface,which enablesyou
to perform instrument operations,view vial statuses,print reports,etc.

FloppyOiskDrive
A floppy disk drive is locatedon the front of the instrumentto enablethe instal-
lation of software updates and to copy important data files to floppy disk.

External?orAs

Ports on the rear of the instrument allow the user to connect an optional Re-
moteAlarm unit and an optional printer.Two other ports are for Field Service
diagnosticuse.

Components of the BACTEC@9050 instrument are selectedand designedto


maintainelectricaland optical integrity throughout the product'slife.All instru-
ments are calibratedat the factory prior to shipment,and should not require
recalibrationduring the usablelife of the instrument,unlesscertain components
are replaced.Calibrationhelpsto insurethat any fluorescentseriesmedium vial
in any stationwill haveinitialand finalfluorescentvalueswithin a specifiedrange.

1,3 Software Overview


The system software presentsa simplifieduser interface on the LCD Display,
with picture icons to representall the functions,operations,setuP Parameters,
and statusconditions(seeSection5 - Referencefor charts of all icons).Routine
systemoperationsare performed by pressingthe teal soft key that corresponds
to the definitionshown on the screen.

There are three basictypes of displays:

MainOtaEue 9creen -When the instrument door is closed,this screen


appears.lt shows the number of vialsthat are positive,negative,ongo-
and stationsthat are in error or anonymous.Alsoshown
ing,available,
are the current date and time and the instrument temperature.Soft-
ware keys allow you to configure the setup parameters,review system
errors, or print the SystemStatusReport.SeeFigure3.

ConfiguraiionOcreens-Accessible from the Main StatusScreen,theCon-


figurationScreensallow you to set the protocol length,the time and
time format, the date and date format, the audiblealarm volume,the
instrumentidentificationnumber,theDVE (DelayedVialEntry) thresh-
old (feature not availablefor use in the USA), to select the desired
languagefor reports,to write data to a floppy disk,and to updatesys-
tem software.

Activity Screene-When the instrumentdoor is opened,software key defi-


nitionsappearthat enableyou to enter new vials,removepositivevials,
remove negativevials,identifyanonymousvials,and resolvestation er-
ror conditions.

1.3.1 Ouilt-in-Teot,(?atent ?ending)


The systemsoftware is designedto continuouslymonitor the electricaland op-
tical performanceof all stationssimultaneously.This functionality,calledBIT (for
Built-in-Test),automaticallymonitors each station every ten minutes for basic
operationalcharacteristics.These tests continuallyverify that signaloutput for
eachstation is within designlimits;this includesboth empty stationsand stations
with ongoingvials.

Two different signallevelsare used to verify operation of the station over the
establishedsignalrange.Testsare performed on dark readings(the output from
the station when its excitation LEDs are off) and on fluorescenceunit readings
(the output from the station when a vial is presentand the excitation LEDs are
on).

Dark readingsare evaluatedto be below a maximum establishedrange.When


they exceed that range,the software declaresthe station in error. High dark
readingsmay indicatea light leak in the cabinetor an electricalfailurewithin the
instrument.

Fluorescenceunit readingsare evaluatedto be within a specifiedmaximum and


minimum rangewhich hasbeen establishedfor the instrumentduring calibration
at the factory.Shouldan out of range readingoccur,the software declaresthat
station in error.Thismayoccur due to an electricalor opticalcomponentfailure.

Additionally,the fluorescencereadingsare evaluatedfor consistencywhile vials


are in stations.lf consecutivefluorescenceunit readingsvary by more than a
predeterminedamount,the station is declaredin error.This feature determines
the stabilityand the acceptabilityofa station for use during protocol.

These protection featuresverify that calibrationhas been maintainedfor all sta-


tions within the system,and insurethat the user is alerted of electronicor opti-
cal changesor failureswhich may be significantenoughto affect results.

The function of BIT can be demonstratedby enteringa vial into the instrument,
and then removingthat vial from the station without scanningit out.The result-
ing error is the responseof the BIT function.Theuser may resolvethe error by
using the resolve errors soft key.

Keye
The four keys that havefixed functions are the UP ARROW (lncrease) Key,the
DOWN ARROW (Decrease)Key,the HOME ROTOR Key,and the SILENCE
ALARM Key.Thesekeysare marked with symbolsrepresentingtheir functions,
and operate identicallyregardlessof the active displayor operation.Six other
keys, the Software (Soft) Keys are teal in color, and have functions that vary
dependingon the active display.Eachdisplayshows icons representingthe cur-
rent Soft key assignmentsat the bottom of the screen.Toperform the function

For all instrumentswith serial numbersfrom 1000- 1695,parts replacements


should be performed by service personnel only. Instrument Power should be
turned off, and the power cord should be disconnected,before beginningany
module replacementprocedure.
The intakefilters on the sidesof the instrument must remain unobstructedat all
times. Restrictedair flow may causeexcessivetemPeraturesin the instrument'
which can affectorganismrecovery and possiblycausehardware malfunctions.
Becauseof its size and weight,two persons should lift the BACTEC@9050 in-
s!rument.
It is mandatorythat all systemusersbecomethoroughlyfamiliarwith all controls
and indicatorsbefore attemptingto operate the instrument.
The door sensor helps protect you from possibleiniury from the movement of
the rotor. Do not tamper with the door sensorin any way,or attemPt to defeat
its function.
Never attemot to insert or remove vialswhen the rotor is in motion.
"UniversalPrecautions"rshould be followed in handlingall items contaminated
with blood or other body fluids.

All portions of the body that could possiblycome in contact with the affected
instrument surfacesmust be completely covered before beginningthe decon-
tamination process.
After the rotor is removed,DO NOT lay it down on either the front or back
sides.(On the rear,the tabs can snap off.) Standthe rotor upright and wedge
both sidesso it does not roll.

WARNINGS/CAUTIONS relating to the use of BACTEC MYCO/F


LYTIC culture vials with the BACTEC 9050 instrument:

BiosafetyLevel2 practice,containmentequipment,and facilitiesare recom-


mended for preparingacid-faststainsand for culturing clinicalspecimens.
For activitiesinvolvingthe propagationand manipulationof Mycobocterium
tuberculosisor Mycoboaerium species grown in culture, Biosafety Level 3
practice,containmentequipment,andfacilitiesare requiredas recommended
by CDC.

Becausean inoculated leakingor broken vial may produce an aerosol of


or other bacteria,appropriate han-
mycobacteria,includingM.tuberculosis

lf an inoculatedvial is found to be leakingor is accidentallybroken during


collection or transport, use the establishedprocedure in your facility for
dealingwith mycobacterialspills.As a minimum,"Universal Precautions"

the rare instancewhere a vial is found to have leaked contents into the
instrument proper,or if a vial is accidentlybroken,turn off the instrument
immediately.Vacatethe affected area.Contact your facility's Safetyor Infec-
tion Control Officer(s).Determine the necessityof turning offor modifying
the settingsof the air handlingunits servingthe affectedarea.Do not return
to the area until any potentialaerosolshavesettled or havebeen removed
by appropriateventilation.Becton Dickinson MicrobiologySystemsshould

Dickinson representativein your area.Guidelinesfor proper handlingof
accidentalmycobacterialcontaminationdue to breakageof culture tubes or

2,1 GeneralRequirements
The BACTEC@9050 instrument should be installedin an area that is free from
undue vibration,direct sunlight,high humidity,dust,temperature extremes,and
corrosiveor explosivevaporsor gases.Thesystemwill operate within specifica-
tions in room temperaturesbetween | 8.3' - 32.0' C (65" - 89.6" F). Relative
humidity should be between l0% and 90% (non-condensing). Clearanceson all
sidesshouldbe at least l2 inches.Environmentswhich exceedthese limits could
adverselyeffect the performance ofthe system components.
The incubatorshould maintainits temperatureto within plusor minus 1.5' C of
the temperature setting (35'C).This accuracycan be assuredonly if the room
temperature meets the requirements given above.

CAUTION
The intake filters on the sidesof the instrument must
remain unobstructed at all times. Restricted air flow
may cause excessivetemperatures in the instrument,
which can afiect organism recovery and possibly cause
hardware malfunctions.

2.4 lnstrument 5etup


2,4.1 General
The BACTEC@9050 instrument shipswith all setup parameterspreset to fac-
tory default values,Before usingthe instrument for blood culture testing,you
should review the instrument setup parametersto see if they are suitablefor
your laboratbry.Theseparametersare describedin Section2.4.2- SetupParam-
eters.and include:

3.3.2 HOMER0TORKey
The HOME ROTOR key is locatedon the right side of the Keypad/LCDDisplay
module,below the SILENCEALARM key.When pressed,it causesthe rotor in
the instrumentto return to the "home" position,with the temperaturestandard
bottles in the l2:00 position.The HOME ROTOR key is active only when the
instrument door is closedand the rotor is in motion.

It is not necessaryto home the rotor prior to openingthe door. However,you


may find it easier to locate vial stations when the rotor is homed, since the
rotor's position then correspondsto the representationin the display.

Note that if vial testing is in progresswhen the HOME ROTOR key is pressed,
those test resultsare discarded.

TheAlarm Indicator is located on the right side of the Keypad/LCDDisplay,at
the bottom.This light flashesyellow whenever the systemencountersan error

The floppy disk drive is locatedon the front of the instrument,at bottom left.lts

At least one aerobic culture vial and one anaerobicculture vial should be pre-
pared.Topreparea vial,removethe plasticflip cap and cleanthe exposedrubber
septum with70% isopropylalcohol.Use a separateswab for eachvial.Inoculate
the vial with the appropriatevolume of sample(refer to the Medio Packoge lnsert
for specificinformation on vial inoculation).

4.2.1 MediaQualityControl
Refer to the specificculture media packageinsert for Quality Control recom-
mendations.

4.4 EnteringNewVials
After specimenshavebeen prepared,they should be scannedand placedin the
instrumentas soon as possible.f.lowever,all BACTEC@ fluorescentseriesinstru-
ments (9240,9120, 9050) have delayedvial positivity criteria that provide for
routine delayscausedby vial transport, overnight lab closures,etc.The system
accommodatesthe following delayconditions:

Before placingvials into the instrument,you should alwaysscantheir barcodes


and placethem in their assignedstationsthrough the vial entry activity.Certain
mediatypes use different positivitycriteria,andthe systemcan only applythese
criteria if the vial label- in which the mediatype is encoded- is scanned(or the
mediatype is entered manually).ln addition,if the vialsare not assignedto their
stationsprior to placementin the instrument,they becomeanonymousvials,and
must be identifiedbefore other states (such as positive,negative,etc.) can be
displayed.Anonymous vial identificationinvolvesthe removaland reinsertion of
vials,whichcan result in an increaseof falsepositiverates.Thereforeanonymous
vial loadinginto the instrument should be minimized.

There are two ways to enter new vialsinto the instrument:

4.5.1 General
Manypositivecultureswill be detectedin the first 24 hours after inoculation,but
ongoing negativevials must still be kept for severaldays to assure maximum
recovery.With the BACTEC@9050 instrument,vialsare typicallyheld for 5 - 7
daysbeforethey are discardedas negative.Eachlaboratory shouldset the proto-
col length basedon its own policiesand conditions.Yourprotocol lengthcan be
from4-7 days.

A subcultureand a Gram stain should be oerformed from each oositivevial.In


most cases,organismswill be seenand a preliminaryreport can be madeto the
physician.Preliminaryantimicrobialsusceptibilityand identificationprocedures
may also be set up from fluid in the culture vials.Theresultsfrom these prelimi-
nary tests should be confirmed by usingstandardizedlaboratory procedures.

The systemallows re-entry of instrument-positivevialsfor up to 3 hours after


their removal(subiectto the conditionsdescribedin Section4.5.3).There-entry
feature resetspositivityroutines,retainsprevioustest readings,and continuesto
test the vial as an ongoingculture.

For a maximumyield of isolates,negativeculturesmay be stainedand/or subcul-


tured during the test period or immediatelyprior to their disposal.

The systemallows you to return a pulled positivevial - if the vial was originally
scannedinto the instrument- for up to 3 hours after removal (or until the vial
goesout of protocol,whicheveris earlier).There-entry feature resetspositivity
routines,retainsprevioustest readings, and continuesto test the vial as an ongo-
ing culture. lf the vial is not returned within the 3-hour re-entry window previ-
ous test resultsare discarded.lf the vial was manuallyentered (by selectingme'
dia type),the systemtreats it as a new vial if it is removed and subsequentlyre-
entered.

The following precautionsshould be observedwhen usingthe vial re-entry fea-


ture. Instrument-positivevials should be subculturedand Gram stained upon
removalfrom the instrument.Eventhough a 3-hour re-entry window existsfor
the return of a vial to an ongoing status,the vial should be returned to the
instrument as soon as possible.Vialsshould remain at room temperature while
they are out of the instrument.

To return a pulled positive vial to the instrument for further testing,open the
door. Pressthe "vial entry" soft key.Next scanthe vial'sbarcode label.Placethe
vial in the indicatedstation (this may differ from the original station).Previous
test resultsare retainedonly in the following circumstances:

+ You originallyscannedthe vial barcodewhen entering it in the instru-


ment.

+ You return the vial within the requiredtime frame.

+ You scanthe vial labelto re-enter the vial.

* You return the vial to the sameinstrumentfrom which it was removed.

lf you do not use the vial entry activity to re-enter the vial,it becomesAnony-
mous at the next test cycle,andthe previoustest resultsare removedfrom the
databaseif it is not identifiedwithin the 3-hour period.

Status Report that provides information about current and recently removed
culture vials.Thereport is dividedinto three main sections:thetop section con-

4.8 ldentlifying AnonymousVials


An anonymousvial is one which is physicallyin a station but has not been as-
signedto that station through theVial Entry activity.lt is very important that
when the systemnotifiesyou of anonymousvials(anonymouscount in Summary
window incrementsand ldentifyAnonymousicon appearswhen door is opened),
you identifythem as soon as possible.Theanonymousstate supersedesall oth-
ers,which meansthat vialsthat are anonymousmay in fact also be positive,out-
of-protocol, in error, etc. However, before any other status can be displayed,the
anonymousvial must be identifiedto the system.

The ldentifyAnonymousfunction enablesyou to locate anonymousvials in the


instrumentand assignthem to stations.

Open the instrumentdoor.

Follow the steps in Figure 15.

Note that becausethe removaland reinsertionof vialscan result in an increase


of false positive rates,anonymousvial loading into the instrument should be

4.9 ?owerFailuresand ManualOperation


The information in the systemis stored in memory until a power failureoccurs.
When a power failureoccurs,thedata is transferredinto non-volatileRAM.Data
in NVRAM is maintainedby a long-lifebattery.After the data is transferred,the
systemis completelyshut down. No vial testing occurs until power is restored.

When power is restored, the times that power was lost and restored are noted
in the system alert list and can be viewed on the display(note that if multiple
power failures occur, only the latest is retained in the alert list). Culture vial
testing resumeswhen power is restored.

For the period of time that power is out, the systemmissesone test readingfor
eachten minutesof power outage.Althoughunlikely,missedreadingscan result
in detectionfailures.Thereforeif testingis interrupted for longerthan forty min-
utes,to ensure maximum recovery it is recommendedthatyou perform a sub-
culture on ongoingculture vials.

You may,at your option,connect the systemto an UninterruptiblePower Supply


(UPS).Use of a UPSmay help prevent interruptionsto testing,or - if the UPS is
connectedto an emergencypower line - the necessityof subculturingvials in
the event of a sustainedoower failure.

6.2.1 Air Filter Replacement


Changeor clean the air filters on both sides of the instrument monthly.lf the
instrument'senvironment is especiallydusty,check the filter more frequently.
Thesefilters must remain cleanand unobstructed;restricted air flow may cause
the vials to reach excessivetemperatures,which can affect organismrecovery
and possiblycausehardware malfunctionsor failures.Thefilters can be cleaned
and reused.See Figure 18.

6 .2.2 Temperature Verification (QC)


A thermometer and specialvial are provided for temperatureverification(QC)
of instrument incubation.lt is recommendedthat the accuracyof the thermom-
eter be verifiedagainsta calibratedlab thermometer to insurethe validityof the
temDeratureverification.

The temperature control circuitry is designedto maintainthe cabinettemPera-


ture at 35" C * 1.5' C. lf your manualreadingis within 1.5"C of the setpoint(35"
C), the controller and heatersare operatingwithin their specifications.

The temperature vial is mounted in a bracket on the insideof the instrument


door (see Figure2).

6.2.4 Aecontamination
A situation requiring biologicaldecontaminationof one or more stations can
occur at any time.The priority in this situationis to first limit the extent of the
contaminationand then to decontaminatethe station or replacethe rotor if
complete decontaminationcannot be accomplished. lf there any doubts that the
affectedstation(s)cannot be completely decontaminated,the rotor should be
reolaced.

The solution recommendedto cleanthe affectedsurfaceshould be at leasta l0


per cent householdbleachsolution.Allsurfacesmust be thoroughlywashedwith
the freshly preparedbleachsolution so the surfacesare "glisteningwet." lf you
are not sure of the extent of the contamination,thoroughly wash the entire
exterior of the Rotor with the freshly preparedbleachsolution.Proceedin the
following mannerto decontaminatea station or Rotor if a vial should break in a
station.

6.3 ModuleReplacement
The BACTEC@9050 instrument has been designedand tested for troublefree
performance.However,in the event of a malfunction,most of the major system
components can be replaced.Proceduresfor removing and replacingcompo-
nents are provided in the sectionsthat follow.

Replacementmodules may be swappedfor failed modules which are then re-


turned to Becton Dickinson.Credit is then appliedtowards the replacement
module.Only replacementparts suppliedby Becton Dickinsonshouldbe usedin
the proceduresdescribedin this section.

circuit board (not the connector).Align the dark blue strand of the flat
ribbon cablewith Pin I of the printed circuit board,and plug in the cable.

Note that in most cases,the dark blue strand will be oriented at the center
of the board.However , this orientation can vary amongdifferentdisk drive
manufacturers.lt is important to verify the location of Pin I on the drive's

To Remove DVE bottles from Bactec:

1. The"Removenegatives" icon, + E , will not appearonthe


displayfor DVE bottles.TheDVE bottle(s)mustbe removed
manually at the time that the bottte(s) would have normally been
flagged as negative" by the Bactec (if the bottle had not required
DVE).

2. Remove DVE bottle(s) from the Bactec at the appropriate date &
time and closethe door to the Bactec.

3. Shortlyafterbottlesaryjgmovedfrom the Bactec,an
