RISK FACTORS FOR ORAL DISEASE FACTORS THAT PROMOTE ORAL HEALTH

ORAL HEALTH HISTORY

❏ Active untreated dental decay ❏ No active decay / all teeth restored


❏ Previous dental decay experience ❏ No or minimal history of dental decay
❏ Deep pits & fissures in teeth ❏ Sealants on back teeth
❏ Severely crowded teeth ❏ Properly aligned and positioned teeth
❏ Poor oral hygiene ❏ Good oral hygiene
❏ Reduced saliva flow / dry mouth ❏ Mouth lining moist with clear saliva
❏ Inadequate fluoride ❏ Optimal fluoride
❏ Poor family oral health ❏ Good family oral health
❏ No family dentist ❏ Visits dentist for routine periodic exams
❏ Last visit to dentist over one year ❏ Visits dentist at least once a year
MEDICAL HISTORY
❏ Poor diet ❏ Healthy balanced diet
❏ Metabolic disease (e.g., diabetes) ❏ Medical condition managed and stable
❏ Infectious disease ❏ Medical condition managed and stable
❏ Neoplastic disease (e.g., leukemia) ❏ Medical condition managed and stable
❏ Medications that affect the mouth ❏ Alternative medication if possible
(e.g., Dilantin, sugary liquid medications)
❏ Gastric reflux ❏ Medical condition managed and stable
❏ Frequent snacks ❏ Occasional snacks: fruits & vegetables
❏ Baby bottle at night for sleep, or at will ❏ Bottles only for routine feeding
CULTURAL / SOCIAL / FINANCIAL STATUS

❏ Poverty, low income ❏ Income higher than 400% of poverty


❏ No or inadequate dental insurance ❏ Good dental insurance coverage
❏ Parents’ education up to 12th grade ❏ Parents’ education beyond 12th grade
❏ Tobacco or alcohol use ❏ No tobacco or alcohol use
