company. The oldest Indian construction company with legacy of 150 years. The first Indian
construction company in middle east in 1970’s. company had been participated in ISO 9001
certification across all projects.

Inspection team consists of one internal auditor, HSE officer, senior manager of site
and 2 safety stewards provided by the company.

EXECUTIVE SUMMARY
This organization the management decided to have zero incident /near miss/accident
at the site. We formed a team to visited all critical operations in work place and the system of
working methods. The specified area as a process to have zero incident rate /near miss and
accident in the work site. The following studies were made to identify the hazards in the site,
Risk assessment to be taken, to formulate risk controls and to give an estimate for controlling
the risk. During our visit to site, there where good practice and positive aspects maintained at
the working place.

 The safety policy was displayed properly in the entire place.


 Presence of Positive safety culture in the organization.
 Everyone knows their roles and responsibilities.
 The Employees are aware of the safe system of work and practice.
 The contract workers were also aware of the method statement and safety policy of
the organization.
 There is a proper line of communication.

Apart from the above there were also some of the critical observation found during this
studies, the same has been brought to the notice to the management as if that is not controlled
then it will end up to accident by which the entire Fabrication process may stop due to which
organization may leads to prosecution notices, increase of insurance premiums, financial loss
business loss and will face the legal problems etc.,

Recommendations:

There are some common precautions to be taken to avoid wrong signal


communication. And they are as follows:
 High visibility clothing to signal man.
 Training on crane signals.
 Competent person for crane operation
 Proper communication arrangements to operator and banks man.

Justify :

If the signal is not given correctly or the operator misunderstood the signal
then there must be a serious accident, damage of raw materials, injury to head, hand
and fatality which results in legal process, loss to property, delay in work, financial
loss, and poor corporate image.
This is against the law of “LIFTING OPERATION & LIFTING EQUIPMENT
REGULATION (LOLER) 1998, ISBN 978 07176 2464 V5. APPROVED CODE OF
PRACTICE & GUIDANCE L113 (LOLER ACOP)”

2. Provision for scaffolding: There exist more chance for entrapment of people on
scaffolding therefore it is essential to have guarding arrangements especially on platform of
scaffolding.

Justification:
If the guarding systems are not installed properly on the scaffolding, then there
might be a more chance for workers entrapment on to fall from height which may
result severe bodily injury and even fatal. And finally results in production loss,
public issues, financial loss, and loss of reputation and legal compliances.

equipment.

 Workers tend to behave abnormally.
Recommendations:
 Relaxation room with proper ventilation and sitting arrangements.
 Provision of pantry for refreshment.
 Purified water and toilet facilities
 Proper Communication systems
Justification:
The employees should be provided with necessary basic welfare facilities in
the workplace by the employers otherwise it will results in slow down of work
process ,stress of people ,health issues , physical abuse, violence, misbehaviors,
slowdown of business, financial loss, and loss of reputation.

5. Number of Fire Extinguishers: Sufficient Number of fire extinguishers must be provided
in work place with good working condition.
Consequences:
 Incapable to mitigate the small fire in the work place.
 Injuries and Fatality to workers.
 Building Damage.
Immediate Control Measure
 Replace the damaged fire extinguishers to the work place.
Recommendations:
 Install sufficient number of fire extinguishers.
 Proper extinguishers types in proper place.
 Sign boards and periodic inspection.
 Proper training has to be given on how to use the fire extinguisher.
Justification:
The employer should be compulsory provided the sufficient number of fire
extinguishers with suitable classes of extinguishers according to the work place. If it
is not followed properly this may lead to incapability in mitigating a small fires,
equipment damage, and fatality resulting in public issues, financial loss, loss of
reputation and legal compliances.
THIS IS AGINST THE “ FIRE EXTINGUISHERS –BS5306 PROVISIONS”
( REGULATION – 3 )AND FROM “ THE FIRE PRECAUTIONS ACT 1971”

4. Fire:
There is open fire seen due to accumulation of diesel stained rags and the static spark may
cause the fire to spread to the chemical storage room which may cause damages to vehicles
and serious injuries to workers
Consequences:
 Fire explosion
 Servere injuries to the workers
 Burning to worker
 Damage to moving vehicle
Recommendation :
 Proper disposal of diesel stained rags and following good housekeeping
 Providing good earthing system
 Adequate number of extinguisher according to work nature
 Providing proper fire fighting equipment
 Putting signage and warning boards clearly in work site
Justification:
Fire is the most serious hazards that causes chain of accident that may lead to injury to
worker, damage to equipment, increased insurance premium, delay in work, man
power replacement, training cost, financial loss and loss of business reputation.

CONCULSION :

The inspection was been carried out at shapoorji pallonji construction site it was been
identified various proper good and usage condition. The unsafe observation and risk, hazard
are been documented. Based on the priority and hazard mentioned various corrective action
have been taken and suggested to the organisation to be followed.
To avoid legal action by the enforcement authority or agency, financial loss due to
direct loss and indirect loss, fuhter more motivating the worker and employer to give
importance to the health and safety to be safe system of work.
Finally the report will be keep aware of management of wheatstone project on
potential arises of any concern at site. All concern have been rectified and improved for long
term solution by reference the recommendation are been provided.
