APPLICATION FOR RECOGNITION TO BE FILED AT THE OFFICE OF THE DIRECTOR FOR STUDENT AFFAIRS AND
WELFARE. UPON COMPLIANCE WITH THE REQUIREMENTS, THE OFFICE OF THE DIRECTOR FOR STUDENT AFFAIRS
AND WELFARE WILL ISSUE A CERTIFICATE OF RECOGNITION (COR). THE CERTIFICATE OF RECOGNITION IS VALID
FOR ONE SCHOOL YEAR ONLY.
