which promotes ICTSI’s best interest and avoid Conflict of Interest when making
decisions and taking actions on behalf of the company;

2. My answers to ICTSI’s Conflict of Interest Disclosure Form are correctly stated to

the best of my knowledge and belief;

Manager/Principal/HR Department and have been determined not to result in any
Conflict of Interest:

source of conflict until ICTSI can determine whether a conflict exists and that
conflict shall be resolved.

5. If any relevant changes occur in my affiliations, duties, or financial circumstances,

I recognize that I have continuing obligation to file an amended Conflict of Interest
Disclosure Form;

6. I am executing this undertaking to attest to the truthfulness and accuracy of the

foregoing as well as for whatever legal purpose this may serve.
