CREATING FEEDBACK FORMS USING GOOGLE FORMS

Feedback This allows your site’s visitor to have their say about
the site’s strengths and weaknesses
Google Forms allow the audience to answer a set of questions
you have set.
 It also a tool used to get user’s feedback.
 These can be used for surveys, feedback, online
registration, and customer care support.

1. Open your browser and go to drive.google.com


2. Sign in or create an account.
3. On the left-hand side of your My Drive page, click
New>More>Google Forms
4. You will be taken to Google Form’s interface:
5. You may now fill out the form with the questions

a. Question Title This includes the question that will be answered by your audience.
b. Form Description This creates a subtext under the questions to give more information about a
question.
c. Question Type This changes the type of questions according to your preference.
These options include:
a. Text /Short Answer a question that can be answered in a short of text
b. Paragraph Text /Paragraph a question that can be answered in a long text
c. Multiple Choice a question that can be answered by only one answer in a set of
options
d. Checkbox a question that can be answered with multiple answers in a set of
options.
e. Choose From A List/ Dropdown similar to multiple choice question but the options
are revealed in a drop-down list.

f. Linear Scale a question that can be answered with a numerical range (e.g., 1-5,
1-10)
g. Multiple Choice Grid a question that contains sub-questions with similar options.
 Date a question that can be answered with a
specified date.
 Time a question that can be answered with a
specified time.
6. To add a new question or item, click on Add Question.
7. Click the Send button. The Send Form dialog box will
appear.

ANALYZING YOUR GOOGLE FORMS RESULT


1. The easiest way to view the result is viewing a summary of
the respondents. Click on Respondents> Summary.
2. You will be taken to a Summary of Responses page. This
particularly useful for viewing your results from one time to
time.
However, after a set amount of time, you may want to use this data in spreadsheet and
eventually create your own charts.
3. To use the data collected through responses, click View Responses in Sheets button found on
the upper part of the page.
4. Choose if you want to create a new Google Sheet as the destination of your response results or if
you want to put in on an existing Google Sheet.
5. Once you click create, you will be taken to a Google Sheet which includes the responses for your
survey. From here you have two options:
a. Save this file locally on your hard drive by exporting it to Microsoft Excel. To do this, click
on File>Download as>Microsoft Excel Spreadsheet.
b. Continue working online using Google Sheets and use the built-in features like creating a
chart for your data (found in Insert>Chart).
6. You may now create chart for your numerical data in either Google Sheets or Microsoft Excel. Note
that you must be online to use Google Sheets. Tip: Google Sheets are automatically saved when
the internet connection is constant.

CREATING A QUIZ USING GOOGLE FORMS

1. Open your browser and go to drive.google.com


2. Sign in
3. On the left-hand side of your My Drive page, click New>More>Google Forms
4. Go to Settings, the Settings dialog box would appear and click QUIZZES button on the top of
the form.
5. Click Make this a quiz button, then click Save.
6. On the Question Title, type “CLASS No. MY QUIZ.” (e.g., B1 MY QUIZ)
7. For the Form Description, type the following “Fill up the following fields and answer the
questions given below.”
8. First, click the form and the Question Type will appear.
9. On the Dropdown menu, choose Short Text for the Class Number of the respondent/s.
Note: The Class Number is served as your question
10. Type Class Number on the Untitled Question field.
11. On the lower part of the form, click the Required button.
12. After that, click Add Question button.
13. For Name and Section, type Name and Section on the separate Untitled Questions fields.
14. Again, make them required.
15. For the questions, click Add Section, to create another section.
16. Click Add Question button, on the dropdown menu choose “Multiple Choice.”
17. On the Untitled Question field, you may start typing your questions.
18. Type the first choices on the Option 1 field, and for the choices 2, 3, and 4, simply click Add
Option to add another options.
19. Click the Answer Key button, the Choose correct answers dialog form would appear.
20. On the right top of the form, you may customize the points for each correct answer. Choose 1
point.
21. When you’re done, click Edit Question to go back to the question.
22. Repeat steps 15 to 22.
23. When done, click Send button.
24. The Send dialog box will appear, type the email address of the recipient of your form.
25. For the subject, type “QUIZ” and Message, type “Answer the Quiz”.

26. Click Send button.
