DPD2 SUMMER—TEAM PRACTICE/DELIVERY MODEL PROJECT

Assignment: Assigned DDS teams (three teams per color group from DPH1) work together to create a
practice model to serve the county/community (from your DPH1 project) in the future.

Objective: To create a practice/delivery model to serve the community and utilize the dental team to
the fullest and in innovative ways.

Deliverable: Poster for the team’s practice/delivery model.

The poster includes—
 Rationale for the practice model/content
 Content—Resources, Process, Outcomes/Systems Evaluation
 Learning from the project
Poster session expectations—
 Explanation of the design
 Question and Answer responses

Posters, explanations, and responses that are clear, organized, and substantive are expected. Be
thoughtful and creative.

Using what you have learned in DPD1 and DPH1, as well as your own experiences and independent
research, consider and apply the following in your poster:

 Financial mechanisms (Fee for service, Minnesota Health Care Plans, sliding fee
scales, insurance critical access dental provider, taxes, grants, donations, etc.)
 Type of practice (e.g., private, HMO staff-model or for profit, FQHC (Federally
Qualified Health Center), group practice, 24/7 dental urgent care, other)
 The dental/interprofessional team—who is on your team (DDS, DH, DT, ADT, DA,
CDHC (community dental health coordinator), CHW (community health worker),
other professionals e.g., nurse practitioner, social worker, physician assistant,
physical therapist, pharmacist…) and what are their various roles? How might you
utilize collaborative agreements/collaborative management agreements
(CAs/CMAs) to benefit the practice and the community it serves?
 Scopes of practice/supervision
 Community outreach/engagement (e.g., to schools, nursing homes, pediatric clinics,
mobile units, teledentistry, counselling centers, other medical settings, home health
services, other)

Poster Session:

Be able to describe your vision for success of the practice/delivery model and how each member of the
dental/interprofessional team will contribute to the practice goals. Consider social, political,
technological, economic and financial mechanisms that affect access, patient outcomes and quality, cost
effectiveness and productivity.
