• Fills and dispenses all requisitions from different nursing care units and special areas
• Checks displays on the shelves and composes requisitions of medicines from the main
pharmacy
• Answers telephone inquiries
• Responsible for the cleanliness of the shelves assigned to her/him and monitors the
expiry dates of drugs in those shelves in accordance with the pull out policy
• Does physical inventory in the assigned shelves every first week of the month;All expiry
items will be withdrawn on the 1st day of the month for sealed items and at the end of
the month of expiry for loose medicines.
• Receives telephone manual posting of medication from OR mini-pharmacy; sends a
reprint of charges to OR information desk nurse and secretary for out-patients under
cash basis accounts
• May be assigned in the Pharmacy Office with the following duties and responsibilities:
• Purchases non-formulary drugs
• Prepares printed out-patients requests from Korean and Japanese Help Desk HMO and
CDUH stock holders in accordance to the guidelines
• Prepares all supply requisitions from the different departments found in Cebu Doctors’
University Hospital and from the different sister companies of Cebu Doctors’ University
Hospital (NGH, SGH, MDH, ODH)
• Attends regular departmental meetings, and be involved in seminars and trainings
pertinent to the profession or self augmentation
• Performs other functions and responsibilities which may be assigned by the immediate
superior
RECEIVING PHARMACIST
• Receives physician’s drug order sheet faxed from the nurse station and checks for any
erroneous, violative, and impossible physician’s order
• Clarifies any vague physician orders by informing the station’s charge nurse
• Records and files those orders designating one medication profile for each patient
• Coordinates with resident physicians, charge nurses and staff nurses for any inquiries or
problems that may arise
• Verifies medication profile versus the medications requested by the nurses before
releasing the drugs
• Does profile review of patients for any Large Volume Parenteral medication that will due
within their shift
• Makes sticker tickets and charges the patient with materials to be used in compounding
the medication
• Answers telephone drug inquiries
• May be assigned in the Pharmacy Office when the need arises
• Attends regular departmental meetings, and be involved in seminars and trainings
pertinent to the profession or self augmentation
• Performs other functions and responsibilities which may be assigned by the immediate
superior
COMPOUNDING PHARMACIST
• Orders from the Receiving Pharmacist, counter-checks the Physician’s Drug Order Sheet,
and creates a Medication Profile for each patient’s Large Volume Parenterals
(Incorporation/Infusions) and Controlled Drugs
• Observes proper Personal Protective Equipment (PPE) before compounding
• Compounds controlled intravenous drugs and medication that needs to be incorporated
or infused in a large volume parenteral preparation under aseptic condition
• Attaches sticker tickets and all necessary auxiliary labels to the prepared medication
affixing his signature on the drug label and sealed for dispensing
• Disinfects the clean bench with 70% alcohol and UV light 15-30 minutes every end of
the shift
• Disinfects the clean bench with Solarsept Solution scheduled every other Saturday for
total eradication of potential disease causing microorganism
• Responsible in maintaining the cleanliness of his working area and the availability of the
materials used in compounding medications
• May be assigned in the Pharmacy Office when the need arises
• Attends regular departmental meetings, and be involved in seminars and trainings
pertinent to the profession or self augmentation
• Performs other functions and responsibilities which may be assigned by the immediate
superior
