Subject: Appointment of Guardian to Withdraw Parents’


Dependency and Indemnity Award

Greetings:

I write today ask for your good office’s assistance in appointing SHEREEN
NOCOM, a close family friend, as my guardian in regards withdrawing my Parents’
Dependency and Indemnity Award (DIC). A copy of Shereen’s valid identification card is
herein attached as Annex “A”.

As a brief background, I was granted monthly payments by the Department of


Veteran Affairs due to my son’s, Renaldo Salvanera, military service with the United
States of America. This DIC award is being deposited through Bank of the Philippine
Islands (BPI) with account number 008384-0052-42 A copy of the letter adjusting my
monthly payment dated 19 November 2012 is herein attached as Annex “B”.

Sometime December 2018, I was hospitalized due to heart and kidney problems. I
was transferred to Philippine Heart Center (PHC) in order to make sure that I get the
best care for my cardiac illness. Ever since then, I was being told to sign indemnity
agreements with BPI for them to issue checks in order to pay for my medical expenses.
My children are no longer residents of the Philippines and I am currently relying on the
DIC awards to pay for my medical expenses. Due to the restrictions in my BPI account,
I can not pay for necessary expenses such as but not limited to caregiver compensation,
proper food and medicine which are not available at PHC among others. The continuous
execution of documents in order for BPI to release and issue checks has cause
unnecessary delays in treatments that I should be receiving immediately.

BPI has informed my counsel and I that I need to appoint a guardian through your
good office so that all the necessary expenses and the medical bills needed to prolong
my life may be paid immediately. According to BPI, my appointed guardian may
withdraw the full amount from my BPI account. Hence, this letter.

Recent developments have compelled me to write to your good office. My kidney


is no longer functioning well and my vitals are no longer stable. I am undergoing
dialysis in a regular basis and I’m afraid that any delay in paying the hospital may prove
fatal to my well-being if no payment is settled when needed. It should be noted that
my failing health makes me bedridden and unable to move around. A copy of my
current medical abstract is herein attached as Annex “C”.

In view of the foregoing, I hereby appoint SHEREEN NOCOM as my lawful


guardian with regard the said BPI account in connection with the DIC award. That she
be allowed to withdraw in my behalf the money deposited in the said account in order to
defray my medical and necessary expenses to improve my wellbeing.

In view thereof, I would like to request for your good office’s assistance by
granting my request to appoint SHEREEN NOCOM as my lawful guardian for the said
purpose.

I hope that the above clarifies the purpose of my request and I hope that the
same would be given due course and be treated with urgency given my current
circumstance.
