1.0 INTRODUCTION
Lutech is about to take up construction of New storage tanks as per API 650 for
storage of petroleum products, namely, TK 8 (Crude), TK 9 (Naptha), TK 10 / TK 11
(both Naptha), TK 12 / TK 13 (both AGO), TK 14 / TK 15 (HFO). Two more Service
Tanks will be constructed on HFO and AGO service namely, Tk 16 and Tk 17
respectively, necessary piping to Tanker discharge line header, BRV loading gantry,
using the existing Pump house area, necessary civil construction for Refining facility
like CDU, Boiler, Heat Exchanger, Furnace etc., integrating Tank Farm / Pump house
with Refining facilities and help erecting refinery facilities under the supervision and
guidance of M/s. V-Fuels, USA as per drawing / lay out, for M/s. Conex Petroleum
Refining Coy for their proposed 10,000 BPD Petroleum Refining at Free Port, Monrovia.
There would be additional activities like integration of New facility as stated above to
existing Conex PST Terminal that includes cleaning of TK 5 & TK 6 and related tie in
work after tank cleaning and gas freeing pipeline and Tanks cleaning done by Conex
PST. There are plan to tie in pipeline of Naptha and Gas Oil from ISBL to OSBL to that
with Mo Gas and Gas Oil Shipping line respectively to facilitate Export.
Shut down shall be required for integration of the existing facilities with the new
facilities which can be done on holidays based on the schedule to be provided by
Conex.
The project poses a challenge in terms of Health, Safety and Environment as the
construction has to be carried out in an existing oil terminal without disrupting day to
day operation of Conex PST terminal.

This document explains the Basic Philosophy, Policy, Actions needed and Methodology
thereof to contain Injuries, loss of properties and damages to environment.
Imperatives of such a plan are to minimize the adverse effects of an accident and
restoration of normalcy at the earliest by providing measures to contain the incident
and limit the consequences.
This Emergency Response Plan adopted will go a long way to control and contain
Injuries, loss of properties and damages to environment, in the event of an Incident,
under various circumstances.
The Lutech Engineering is committed to this Emergency Response Plan to put into
Practice in letter and spirit.

It is expected that Lutech Engineering Project site has an established and well-
rehearsed plan of responses to an Emergency situation which may arise out of various
activities undertaken. It is our intention to provide guidance to make the Emergency
Response Plan effective.
Site Management shall establish and train site personnel regarding emergency
response procedures.

8. Secure and isolate incident site. Do not move anything that does not have to be
moved, only things to assist the injured or make the area safe. Make note of
those items that must be moved. For major incidents site emergency shutdown
is required

Sub-contractor employees
10. Keep only those on site who are essential in the recovery process. Release
those who are not needed and require them to leave the site

Project Management will ensure the Emergency Action Plan is communicated to all
workers during orientation. Specific emergency procedures and emergency phone
numbers will be posted in site Office, near all telephones and on project bulletin
boards.

• Evacuation
Personnel should de-energize / shut down tools and equipment and observe their
work area for fellow workers in need of assistance.
Report any hazardous conditions that are known to exist within the work area to
his supervisor.
A site plan drawing will be developed for project’s evacuation plan. This drawing
will clearly identify the following:
o Building footprint
o Primary and secondary assembly areas
o Exits
o Fire alarm pull stations or air horn locations
o Site telephones
o Stairs
o Fire extinguishers
o Lutech project office
o First aid kit locations
o Emergency numbers

• Medical Emergency
During the safety orientation, workers will be given information on how to summon
medical assistance in case of a medical emergency. Workers should know the
following information:
When reporting a medical emergency, the worker will state their name, the nature
of the emergency, the severity of the emergency and where assistance is needed.
A worker may be required to meet medical personnel and guide them to where the
emergency is located.
Please remember:
➢ Provide First aid/ Medical assistance including Hospitalization at the
earliest to the injured.
➢ Do not move an injured worker before medical assistance arrives unless
further injury is possible.
➢ Unconscious worker inside confined space only to be rescued by experts/
trained personnel.
• Fire Emergency
In case of a fire, workers will evacuate their work area immediately and report to
the pre-determined assembly area.

• Severe weather
Workers will immediately evacuate their work area upon hearing the alarm or
being notified of the emergency and ordered to evacuate. No worker is exempt
from evacuation even if the evacuation is a drill.
Workers are required to report immediately to their designated assembly point and
be accounted for. Failure to report may cause another to risk danger in an effort
to search for missing worker. Do not leave the project without prior authorization
from front-line supervision.
A Lutech employee will call for assistance from Terminal Manager. Any other
Chemical or Gas leakage or spillage will be identified in PMP

In order to control and manage any incident on a Lutech Engineering project the
following measures will be followed:

o All employees working on Lutech Engineering projects will follow the Return to
Work Policy in this manual. Each subcontractor shall be responsible for ensuring
their employees comply with this manual.
All means necessary will be provided to the injured employee to ensure a rapid
recovery and return to work processes. As a part of this plan, the injured employee
will be provided the opportunity to return to work as soon as possible on light duty
and transition to full duty upon medical full release from care.
Lutech Engineering, along with the subcontractor supervision, will work closely
with all medical doctors, and specialists in order to ensure close co-operation from
all parties in returning the injured employee back to full and normal duties as soon
as possible.
Lutech Engineering shall monitor all medical and other costs involved from the
injury to minimize and control the cost of providing necessary medical treatment.
As soon as an injured person has been removed from the scene, have someone
immediately secure the scene without disturbing physical evidence and take names
and phone numbers of all witnesses. If necessary, administer first aid until help
arrives. Strategically place employees to direct the emergency response team to
the incident. For emergencies requiring evacuation, each project will develop a
site-specific plan.

• Incident Reporting:
All incidents covered under Level-I will be maintained by the entity for inspection
whenever called for inspection and Level-II and Level-III will be reported to the
Project Director / MD in the format specified including near miss incident. The
above report will be submitted within 48 hours after occurrence of the incidents or
any other reason triggering major incident. The Incident Report Format is given as
per Appendix C

Investigation report of all major incidents will be submitted to the Project Director
/ MD. An incident shall be treated as Major if any of the following occurs;
A Basic Root Cause Analysis (RCA) involves a closer look at four criteria that may
have been a factor in the development of the conditions that led up to an incident.

existing Conex petroleum Refining Coy’s Terminal, wherein Terminal Manager will
coordinate with Port/ Govt. Authority for Level III Emergency.

Note:
1. For fires of Electric type, use only CO2 or dry chemical fire extinguishers to
extinguish fire on or around electrical equipment. Do not use water or foam.
2. Do not extinguish a gas fire until the source of fuel has been cut off to avoid an
explosive mixture being formed.

• Sound the local siren / alarm to raise the emergency / fire alert. The name of
informer if any, time of alarm, time of turnout of fire / emergency, Crew &

• Inform the Security staff at main entrance of the CONEX Terminal over Walkie-
talkie for necessary assistance that will raise Emergency / Fire alarm by Siren.
• Inform the Project Manager, Terminal Manager, QAP Manager, Site Engineer, HSE
Supervisor over phone.

• Attend all telephone calls and take any further action as advised by
Project Manager.
• Ensure that the Gate is always manned
• Keeps an account of People / Vehicle coming in/ out. Only Emergency related
people and Vehicle should be allowed.
• During Off shift, inform State Emergency service: 911 for assistance, if needed.
Generally, TM/ PM will coordinate with State Emergency services.
Responsibilities of Fire / Emergency Combat Team: Team Leader- Site engineer (Civil
/ Mechanical / Electrical)
• Fight fire with 9 Kg DCP / 4.5 Kg CO2 Fire Extinguisher available nearby.
• Team leader to guide combat team for speedy action and keep members on
upwind
• Team leader to ensure availability of back up men and firefighting materials.

Role of key personnel is clearly defined to avoid confusion and to meet the emergency
effectively. The Project Manager will be responsible for effective control of an
Emergency at site. Since, the site is located within the Terminal, the Conex PST
Terminal Manager, being the custodian of entire premises has overall responsibilities
for effective control of Emergencies in Conex premises including the Project site.
The Project Manager will seek assistance from Terminal manager for any Fire /
Emergency arising out of Project activities, that includes excavation, erection,
installation, Hot work (welding / gas cutting / grinding / sand blasting / heating etc.),
working at height, use of Mobile Plants, storing / handling of fuel for DG / Mobile
Plants, Painting, transportation of materials, transport Vehicles etc. Such assistance
includes Fire Extinguishers, Foam, Fire water hydrant points, Water / foam Monitors
on FW network, operating FW pumps and Safety equipment’s like Self Contained
Breathing Apparatus, Explosimeter, Fire Proximity Suit, Resuscitator, Stretcher, Oil
containment Boom, transport vehicle etc. and expertise in terms of competent and
experienced man power in Fire combating, Rescuing, First Aid etc.
The onsite Emergency Organization Chart is given as under and will be applicable
during on shift or working hours, wherein Terminal Manager / Project Manager
assumes responsibility. A separate Organization chart is furnished which covers off
shift Emergency Organization Chart wherein, Security Supervisor / Security
Commander assumes responsibilities.

Security Commander is to inform Project Manager over phone about any Emergency
arising during off shift. In case of emergency Level II and Level III, the Terminal
Manager / Project Manager are to take Charge of the situation. In other words,
Security is responsible for Level I emergency handling during off shift.

Security Commander is to inform Project Manager over phone about any Emergency arising during off shift. In case of emergency Level II and Level III,
the Terminal Manager / Project Manager are to take Charge of the situation.

Fire may take place due to leak / rupture of pipeline carrying product in Tanker
Discharge line. Fire in Tanker, Exchange pit, hammering of defective valve or use of
sparking tool. In the event of such situation following action shall be taken:
• Fire has to be attacked promptly with the nearest available extinguishers before it
has a chance to spread. Alarm should be raised.
• All unloading operations, if any, should be stopped at once. Pumps have to be
turned off, hoses be removed, and all the valves should be closed without much
delay.
• Tanker Master to be informed for stop pumping
• DCP coupled with sand should be discharged to extinguish the fire.
• Water monitors / jets need to be used to cool the Tanker and adjacent pipelines,
as needed. Apply foam to extinguish the fire
• Inform Port Authority, Fire Service / PFSO for assistance
• Inform China Union to enable them to take precautionary measures including
stopping of operations.
• If the pipeline leaks, then foam should be applied gently at the affected area. In
the same time, the above steps shall also be taken care of.

• A fire at the vent will not normally flash back into tank and explode if the tank
contains product.
• Close all valves since any removal or product will result in air being sucked inside,
resulting in flash back and explosion.
• Close manhole covers of other tanks if they are open.
• Cool the tank with water jet.
• Smother the fire by putting on manhole cover or cutting off flow of vapors with
damp rags, gunny bags.
• Fire at small leak in pipelines must be attacked promptly with the nearest fire
extinguishers.
• Shut-off the flow of Oil in the lines by closing valves and by stopping pumping, if
any.
• Cover the Oil pool with sand and build up the sand / earth so as to cover the leak.
• Put foam on the burning oil pool.
• Build earth dykes around the oil pool to prevent spreading of burning oil.
• Drain out the contents of Pipeline between closed valves to avoid bursting.
• Cool the adjacent structures.

• Stop pumping.
• Stop flowing of Oil from drain. Keep oil within limited area. Build earth dykes.
• Close line valves.
• Dig pits to collect oil.
• Salvage the product.

• All structure / buildings have been designed to withstand cyclones, storms and
hence not much damage is anticipated.
• No temporary structure / shed during Storm / Cyclone season at Project site
• Switch off all connections.

• Bring all vehicles to a halt and ensure that hand break is applied, kept at neutral.

Bomb Plantation
• The person noticing an unidentified object something like bomb should bring it to
the notice of the nearest available officer.
• The Engineer / Supervisor should observe the same from a distance and raise alarm
to declare emergency in the terminal.
• The area should be cordoned off and nobody should be allowed to go near the
object.
• PFSO, Police and Fire Service to be informed as per instruction of Works Controller
• On arrival of Police and fire service, rescue team in charge to bring them to the
spot after the bomb is diffused or found to be hoax, all clear signal should be
given.

• In case of Tank overflow or leakage in dyke area, ensure that dyke valve and fire
break wall valves are closed
• Use foam for fire or blanket spilled oil by foam
• Adjacent area or tanks to be cooled as needed.
• Drain out the Oil safely in empty drums / tankers or collect oil through Oil Water
Separator.
• Repair of mainline, restoration of fittings / tanks as per requirement.
Evacuation Plan
• When the emergency message is received, all personnel should ensure that nearby
personnel are aware of the emergency, quickly shutdown operating equipment,
close the doors and exit the building using staircase / work site.
• All personnel should know where primary and alternate exits are located, and be
familiar with the various evacuation routes available.
• When officially told to evacuate, leave by the nearest exit and alert others to do
the same.
• Once outside, move to the assembly point designated for the plant/ Project site.
Stay there until an accurate head count has been taken.
• Keep streets and walkways next to exits clear for emergency vehicles and
personnel. Keep the street side of buildings clear so that emergency equipment
can reach buildings / work site.
• Do not return to an evacuated plant / building until & unless directed to do so by
the Fire & Safety / Emergency Department personnel.

In order to meet all possible emergencies, adequate number of resources and systems
has been provided as per identified Risk level at the CONEX PST Terminal and also in
compliance with requirements as per NFPA 30.

The equipment’s like Excavator, Mobile Crane, Mobile Elevated Boom / Platform,
Front Loader, Dumper, Welding, Gas Cutting machines at the Project sites generally,
whose services can be sought during Emergency.
Windsocks:
Windsocks are installed for knowing wind direction during emergency.2 numbers of
Wind Socks have been installed at strategic locations.
There are 3 number of First Aid trained personnel available with Terminal / Project
site. One number of First aid Box provided with Project Site Office apart from two
available with PST Terminal.
First aid is the provision of initial care for an illness or injury. It is usually performed
by trained personnel to a sick or injured person until definitive medical Treatment can
be accessed. Total 3 No. of employees underwent First aid training program.

Heat Exhaustion
Fatigue, irritability, headache, feeling of faintness, cold, clammy skin and a lot of
perspiration
What to do:
• Place victim in lying down position with head lower than the rest of the body.
• Loosen clothing around neck
• Ensure plenty of cold air
• Sprinkle face and chest with cold water
• Put smelling salt or ammonia to nose
• Rub limbs towards body
• Give stimulates which victim can swallow.

• Place several layers of sterile gauze over the burn; saturate with 5 per cent tannic
acid solution if available and apply bandage.
• For serious burns on the body remove all clothing carefully and treat as per first
treatment.
• If there are signs of collapse keep the victim warm and give sips of warm coffee or
warm tea if not unconsciousness.

5. Investigation report of all Major incidents will be submitted to PNGRB. An incident will be
treated as Major if any of the following occurs;
6. Loss time Incident will be monitored till the affected person joins duty. In case the
affected person is yet to join the duty, then the status of report submitted will be
preliminary. Final report against the same incident will be sent once he joins duty and
the man - hours lost are known.
