AFFIDAVIT OF WRONG ENTRY

KNOW ALL MEN BY THESE PRESENTS:

I, JOEY A. KIAMCO, Filipino, of legal age, married, and a resident of Argao,

Cebu, after having been sworn to in accordance with law, do hereby depose and say:

1. That I am the Operations Manager of Eversure Maxim, Inc. with principal

office address at 2nd Floor, One Blue Rhino Corp. Bldg., H. Abellana St.,
Canduman, Mandaue City;

2. That the names and addresses of the ORs and CRs of our clients’
motorcycles, whose names are provided for in the attached list and made an
integral part of this document, were all indicated as “-“ and “Bonifacio St.,
Baguio City of Benguet Cordillera Admin, Region” instead of their actual
names and addresses;

3. That I inadvertently caused the said wrong entries in the DIY of the Land
Transportation Office (LTO);

4. That I am executing this affidavit to attest to the truthfulness and veracity of

the foregoing facts and for whatever legal purpose this document may serve.
