We are informing you that the lease on the referenced property will expire
on June 25, 2019. You are a valued tenant. Because of this we would like to enter
into a new lease on the referenced property for a term of another one (1) year. A
contract of lease is attached to this notice.

All of the terms of the original lease will remain in effect, including the
rental amount and the period of the said lease agreement. If you decided to renew
your lease with us, please inform us or sign the lease and initial at each and every
page of the said contract in which the new contract of lease must be returned to
the undersigned within 30 days from receipt of this notice. Should you wish not
