2019 AFPMBAI GRAND RAFFLE PROMO

I. GEOGRAPHICAL SCOPE AND DURATION

This NATIONWIDE promo will run from JUNE 15 TO NOVEMBER 30, 2019

II. MECHANICS:

1. The promo is open to ALL REGULAR UNIFORMED SERVICES MEMBERS and

ASSOCIATE MEMBERS of the AFPMBAI, as defined in attached Definition of Terms
(Attachment A)

2. Participants must be Filipino citizens, at least 18 years of age, and residing in the
Philippines.

3. AFPMBAI Members can acquire raffle entries and scratch cards by applying for the
following:
a. Insurance Plans- MBAI Protek, 10-Year Endowment, Saver Protection Plus and
College Education Plan
b. New Loans- Salary Loan, Policy Loan, Equity Loan, Members’ Educational
Assistance Loan (MEDAL) and Expanded Salary Loan

c. Housing /Construction/Mortgage Loans

d. Loan Renewals

Note:
 Members who are automatically signed up for MBAI Protect will also
automatically be given a raffle entry but will not be entitled to scratch cards.

 Candidate soldiers for AFP and its counterpart ranks for other branches of
service are not eligible to join this raffle/scratch card promo.

4. Upon sign-ups for insurance plans/new loans/housing loans, members are entitled to
the following:

5. All insurance and loan applications will be subject to a verification process conducted
by the SBUs, to ensure that these applications have completed underwriting
requirements and/or loan applications are eligible for new loans, housing construction
or mortgage loan. Each verified and approved application from June 15 to November
30, 2019 is equivalent to one electronic raffle entry.

6. All verified and approved applications will be submitted by the different Sales
Business Units (SBUs), with the following information per electronic raffle entry:

6. The Official Sales Reports should be submitted to Marketing Division, signed by the
respective SBU Heads, on the following reporting dates:

7. A confirmation receipt shall be immediately sent to the SBUs upon receipt of the
Official Sales Report.
 The data gathered shall be stored in an in-house database and shall be subject to
an electronic raffle draws on September 08, 2019 (Friday) and December 18,
2019 (Wednesday), both at 03:00 PM.
 There will be a total of eighteen (18) raffle draws corresponding to the number
of prizes available for the Members.

III. RAFFLE PRIZES

1. The raffle entries will vie for the following prizes:

 Grand Prizes Two (2) Winners Two (2) Brand New Suzuki APV
 Major Prizes Six (6) Winners Six (6) Honda Wave 110 Alpha
Motorcycle
 Minor Prizes Ten (10) Winners Ten (10) Oppo F7 Mobile Phones

2. The Scratch Cards will entitle lucky winners to the following on-the-spot cash prizes:

 Two Hundred (200) Winners Two Hundred (200) prizes of P1000 each
 Three Hundred (300) Winners Three Hundred (300) prizes of P500 each
 Seven Hundred Fifty (750) Winners Seven Hundred Fifty (750) prizes of P200 each

Note:
 Scratch card prizes are redeemable at all AFPMBAI branches nationwide.
 Each scratch card entitles members to a chance to win instant cash prizes.
Members can scratch out the four scratch portions of the card to reveal if they
won any instant cash prize. All four portions must contain the same amount to
be considered winners of P1000, P500 or P200.
 AFPMBAI will shoulder the 20% tax for all the raffle prizes.

3. All raffle prizes are non-transferable and non-convertible to cash.

IV. DRAWING OF WINNERS

1. Each raffle entry will be assigned a raffle number to serve as electronic raffle entry.

2. The electronic raffle entry shall be raffled off using an E-Raffle Software developed
by IT Division of AFPMBAI.

3. The draws shall be witnessed by an AFPMBAI Internal Audit representative and DTI
representative. Drawing of winners will be documented. The raffle draws will be
held on September 08, 2019 (Friday) and December 18, 2018 (Wednesday) at 3:00
PM, at AFPMBAI Head Office.

  Raffle entries that were not picked as winners during the first raffle draw will still be
included as entries for the second raffle draw in December 18, 2019.

 Winners of the first raffle draw will no longer qualify for the second raffle draw with
the same transaction. Nevertheless, qualifies if he/she has another transaction
within the sales period.

V. ANNOUNCEMENT OF WINNERS

1. Winners will be notified via SMS, e-mail, call out of a representative and registered
mail from the Marketing Division.

2. Names of the winners will also be posted on AFPMBAI’s Facebook Page and Official
Website.

VI. AWARDING OF WINNERS

1. Raffle prizes must be claimed within sixty (60) calendar days from receipt of
notification from AFPMBAI. Otherwise, the prizes shall be forfeited and retained by
AFPMBAI with prior DTI approval.

2. Winners of scratch card prizes will be given up to sixty (60) days upon end of promo.

3. Upon notification of winning, the Marketing Division will identify on where the
member/ winner can claim their prizes through the following locations:

GMA winners – Marketing Division, 4/F AFPMBAI Building, Col. Bonny

Serrano Road cor. EDSA, Quezon City

PROVINCIAL winners - nearest AFPMBAI Branch or Extension Office

 VII. To claim prizes, winners must bring any two of the following government IDs:
Philippine Passport Driver’s License Service ID
Voter’s ID Postal ID
PhilHealth Card TIN Card

Additional IDs acceptable (for Associate Members only):

SSS Card
GSIS Card

VIII. In the event that the member is unable to claim the prize, an authorized representative
can claim in behalf of the member but shall be required to present the following:

1. Two valid government IDs of the winner/member

2. Original copy of the Winner’s authorization letter
3. Two valid government IDs of the authorized representative

IX. DISQUALIFIED PARTICIPANTS:

AFPMBAI’s employees, suppliers, contractors, vendors, agencies and consultants,

including their relatives up to second degree of consanguinity or affinity, are disqualified
from joining the promotion.
