INTRODUCTION

1 INTRODUCTION

1.1 The true profile TMJ system

This program enables true profile exposures of the


temporomandibular joints to be taken with the Proline
XC panoramic and DIMAX3 digital X-ray system.

Long axis of condyle


Before taking a true profile TMJ exposure determine the
angles at which the long axes of the condyles are from
the transcranial line. To do this take a submental vertex
exposure of the patient and measure the condyle
angles.

Condyle angle
If the condyle angles are the same you can take a single
Transcranial line exposure of both condyles. If the angles are different
you must take a separate exposure of each condyle on
the same image. If required you can take an exposure
of one condyle only.

NOTE If the patient has a large head or a long


mandible the cassette or the sensor may touch
their nose during the exposure cycle. This will
not effect the exposure result, as there is no
exposure at that point in the rotation cycle, but
the patient should be advised.
Film-based unit:
If your unit is equipped with the PLANMECA
AUTOPRINT film marking system the jaw size and
anterior shape indicators, that are printed on the film
when a panoramic exposure is taken, will be replaced
with the selected condyle angle/angles and left and right
indicators.

To select the TMJ True Profile program, touch the Prog.


field on the main display.

The Select program type display appears. Select the


program type True Profile TMJ.

Select the desired True Profile program and touch OK.


The main display will appear again.

Insert the special true profile TMJ chin support into the
slot in the patient positioning mechanism. Note that it is
Special true profile shorter than the standard chin support.
TMJ chin support

Touch the Entry field to move the rotating unit to the


entry position.
Touch the temple support field to open the temple
supports if they are not already open.

Touch either of height fields to adjust the height of the


vertical carriage until the opening in the chin support is
approximately level with the patient’s mouth.

Ask the patient to remove any spectacles, hearing aids,


jewellery, and, if necessary, dentures.
Ask the patient to grasp the patient handles, and press
his or her lips against the chin support. The patient’s
nose should rest on top of the support. If you wish to
take an open jaw TMJ exposure ask the patient to open
his or her mouth.

Touch the temple support field to close the temple


supports.

Touch either of the Layer field to switch the three patient


positioning lights on.

Position the midsagittal plane and the Frankfort plane in


the same way as they would be positioned if a normal
TMJ exposure were being taken. Refer to the Proline
XC X-ray unit’s User’s Manual).
Ear post Gently place the ear post, at the end of the TMJ
positioning ruler, into the patient’s auditory meatus.
Position the ruler so that the long arm is parallel to the
ground and the median plane. Do not push the ruler
against the patient’s cheek.

POSITIONING THE PATIENT

Use the Layer fields to drive the patient positioning


mechanism to number 9 on the TMJ positioning ruler.
i.e. the focal trough light coincides with the number 9 on
the TMJ positioning ruler.

Touch either of the Layer fields.

The Angle fields appear. Touch either of the fields to


adjust the required angle of the True Profile exposure.

After selecting the angle, touch Ready to take an


exposure.

Make sure that you have selected the correct True


Profile program (both).

Touch and hold either of the Layer fields to move the


patient positioning mechanism, and patient, backwards
or forwards until the required position is reached.
Touch the Layer field to activate the Angle field. Adjust
the angle with the Angel fields.
NOTE If you can not get the required angle, you have
to change the patient’s head angle, see section
7 “PATIENTS WITH LARGE CONDYLE
ANGLES” on page 11.
NOTE Before an exposure the patient must have been
selected in the Dimaxis software, refer to the
Dimaxis user’s manual, section 2.2 “Selecting
Patient” on page 7.
Touch the Ready field to drive the rotating unit to the
ready position. The indicator light will come on.
Digital unit: Before an exposure the patient and
panoramic exposure mode must be selected in the
Dimaxis program, refer to the Dimaxis user’s
manual.The unit is ready for exposure after the Waiting
for Exposure window appears on the PC screen.
NOTE The ready indicator light will be flashing while
the PC is preparing itself for the exposure.
Move to a protected area. In case you do not have a
protected area available, move at least two meters
(seven feet) from the X-ray. Protect yourself from
radiation.
NOTE Maintain audio and visual contact with the
patient and unit during exposure. If the rotating
unit stops moving during exposure, release the
exposure button immediately.

Hold the exposure button down for the duration of the


exposure. The rotating unit will move through one
exposure cycle. During the cycle the radiation warning
light will come on and you will hear the radiation
warning tone.
When the rotating unit has completed the cycle, the
temple supports will automatically open and the patient
can be guided from the unit.

You can now either exit the True Profile TMJ program,
or touch the Entry field to drive the rotating unit to the
entry position to take another true profile TMJ exposure.
Digital unit:
After the exposure has been taken the image is shown
on the PC screen on the New panoramic image window.
The image must be accepted by clicking OK. Note, that
only the accepted images will be stored in the
database. Refer to the Dimaxis user’s manual, section
5.2 “Taking a Panoramic Exposure” on page 46.

Film-based unit:
Remove the cassette from the cassette carriage after
the exposure and process the film.

ANGLES ARE DIFFERENT
Note that this is a double exposure and the rotating unit
will travel through two exposure cycles.
Make sure that you have selected the correct True
Profile side (left/right) on the Select program type
display.

True Profile TMJ, left will take an exposure of the


patient’s left-hand condyle and then an exposure of the
right-hand condyle. True Profile TMJ, right will take an
exposure of the patient’s right-hand condyle and then
an exposure of the left-hand condyle.
Touch and hold either of the Layer fields to move the
patient positioning mechanism, and patient, backwards
or forwards until the required position is reached.
Touch the Layer field to activate the Angle field. Adjust
the angle with the Angel fields.
NOTE If you can not get the required angle, you have
to change the patient’s head angle, see section
7 “PATIENTS WITH LARGE CONDYLE
ANGLES” on page 11.
NOTE Before an exposure the patient must have been
selected in the Dimaxis software, refer to the
Dimaxis user’s manual, section 2.2 “Selecting
Patient” on page 7.

Digital unit: Before an exposure the patient and
panoramic exposure mode must be selected in the
Dimaxis program, refer to the Dimaxis user’s
manual.The unit is ready for exposure after the Waiting

Film-based unit: The cassette is driven to the Ready
position.
NOTE Digital unit: The ready indicator light will be
flashing while the PC is preparing itself for the
exposure.

Move to a protected area. In case you do not have a


protected area available, move at least 2 meters (7 feet)
from the X-ray. Protect yourself from radiation.
NOTE Maintain audio and visual contact with the
patient and unit during exposure. If the rotating
unit stops moving during exposure, release the
exposure button immediately.
Hold the exposure button down for the duration of the
exposure. The rotating unit will move through one
exposure cycle. During the cycle the radiation warning
light will come on and you will hear the radiation
warning tone.
At the end of the exposure cycle the rotating unit will
return to the ready position and the program name will
automatically change. If you originally selected the left
program, the right program will appear on the display
and vice versa.
Digital unit: The image of the first exposure will appear
on the screen. After this the Waiting for Ready window
and then Waiting for Exposure window appear on the
screen.
Touch either of the Angle fields to select the angle of the
patient’s other condyle. The angle will appear on the
display.
Touch the Ready field to drive the rotating unit to the
ready position. The “Waiting for Exposure” window will
appear on the PC screen.
Move to a protected area. In case you do not have a
protected area available, move at least 2 meters (7 feet)
from the X-ray. Protect yourself from radiation.
NOTE Maintain audio and visual contact with the
patient and unit during exposure. If the rotating
unit stops moving during exposure, release the
exposure button immediately.

Digital unit: If you have taken two exposures, the
images are automatically joined and shown on the
screen after exposures.
After the exposure has been taken the image is shown
on the PC screen on the New panoramic image window.
The image must be accepted by clicking OK. Note, that
only the accepted images will be stored in the
database. Refer to the Dimaxis user’s manual, section
5.2 “Taking a Panoramic Exposure” on page 46.
Film-based unit: Remove the cassette from the
cassette carriage after the exposure and process the

Make sure that either exposure program True Profile,


left or right is selected. When you want to take an
exposure of the patient’s left-hand condyle select left
and when you want to take an exposure of the patient’s
right-hand condyle select right.
Touch and hold either of these fields to move the patient
positioning mechanism, and patient, backwards or
forwards until the condyle angle for the side you have
selected appears on the display.

Touch and hold either of the Layer fields to move the


patient positioning mechanism, and patient, backwards
or forwards until the required position is reached.
Touch the Layer field to activate the Angle field. Adjust
the angle with the Angel fields.

NOTE If you can not get the required angle, you have
to change the patient’s head angle, see section
7 “PATIENTS WITH LARGE CONDYLE
ANGLES” on page 11.

In the true profile TMJ program the place of the focal


trough varies depending on what angle is selected.
Patient with a large jaw or a large condyle angle has not
enough patient positioning mechanism movement.
Therefore you may not be able to get the required angle
without changing the patient’s head angle.

Turning the patient’s head 25 mm decreases the


condyle angle by 10°, and turning the head 35 mm
decreases the angle 15°. These points are marked to
the chin support.
For example:
The patient’s condyle angle is 32°, but the maximum
angle that can be reached is 24°.

By turning the patient’s head 25 mm the angle can be


decreased by 10° to the angle of 22° (note the markings
on the chin rest). Selecting the angle 22° will get the
total angle of 32°

When turning the patient’s head make sure that both of


the condyles are equal distance away from the
midsagittal light plane. Otherwise the condyles are
magnified unequally. Because it is very difficult to
determine the distance from the centre line to the
condyles, do not trust the constant magnification factor
1.2 in these cases.
