Review question
What are the physical, psychological and professional consequences for employees with burnout as
analyzed in longitudinal studies?

Searches
We will search the following electronic bibliographic databases: PubMed, Science Direct, PsycINFO,
SciELO, LILACS and Web of Science with these terms:
a) PubMed: ((burnout[Title]) AND (longitudinal[Title/Abstract] OR prospective[Title/Abstract] OR
cohort[Title/Abstract] OR case control[Title/Abstract] OR case-control[Title/Abstract] OR "follow-
up"[Title/Abstract] OR "follow up"[Title/Abstract])). b) Science Direct: TITLE(burnout) and TITLE-ABSTR-
KEY(longitudinal OR prospective OR cohort OR case control OR case-control OR follow-up OR "follow up").
c) PsycINFO: Title: burnout AND Abstract: longitudinal OR prospective OR cohort OR case control OR case-
control OR follow-up OR follow up.
d) SciELO: burnout and longitudinal OR prospectivo OR coorte OR caso-controle OR caso controle OR

Types of study to be included


The included works must be original studies with quantitative data, published in complete article format in
journals with an editorial board, which deal with burnout and which have a longitudinal study design (cohort
or case-control) and which submit data on the consequences of individuals in burnout, and which seek to
analyze burnout as an exposure (risk factor).

Physical, psychological or professional consequences (outcomes) of burnout (exposure).

Participants/population
Employees.

Intervention(s), exposure(s)
Burnout is a syndrome of emotional exhaustion and cynicism that occurs frequently among individuals who
do ‘peoplework’ of some kind. A key aspect of the burnout syndrome is increased feelings of emotional
exhaustion. As their emotional resources are depleted, workers feel they are no longer able to give of
themselves at a psychological level. Another aspect is the development of negative, cynical attitudes and
feelings about one’s clients. A third aspect of the burnout syndrome is the tendency to evaluate oneself
negatively, particularly with regard to one’s work with clients.
Workers feel unhappy about themselves and dissatisfied with their accomplishments on the job.

Comparator(s)/control
Cohort or case control. A group of healthy individuals.

Data extraction (selection and coding)


This phase has not been completed. Titles and/or abstracts of studies retrieved using the search strategy
and those from additional sources will be screened independently by two review authors to identify studies
that potentially meet the inclusion criteria outlined above. The full text of these potentially eligible studies will
be retrieved and independently assessed for eligibility by two review team members. Any disagreement
between them over the eligibility of particular studies will be resolved through discussion with a third
reviewer. Extracted information will include: lead author, title, publication year, journal, venue of the study
objectives, the study population characteristics, follow-up, instruments used in data collection, study design
(case-control or cohort), dependent variables and independent , used statistical analysis and control
variables (if any), main findings and identified constraints, classification of outcomes in physical,
psychological or professional assessment score of methodological quality and finally quote from conflicts of
interest.

Risk of bias (quality) assessment


One author will assess the risk of bias in included studies by considering the following characteristics:
1. The size of the final follow-up sample is greater than 100 persons.
2. The exposed and unexposed cohorts belong to the same community;
3. The determination of the exposure (independent variable) is given by insurance records or structured
interviews;
4. The determination of the outcome (dependent variable) is given by insurance records or structured
interviews;
5. The outcome of interest is investigated at baseline and excluded from the longitudinal analysis;
6. Follow-up losses are described;
7. The main adjustment factors in the analysis are selected: age and sex/gender;
8. Any additional adjustment factors are selected: education, marital status, occupation, depression or health
aspects;
9. Uses measures of association between dependent and independent variables.

Strategy for data synthesis


We will provide a narrative synthesis of the findings from the included studies, structured around the
physical, psychological or professional consequences of burnout.
