1) Initial Risk
• Severity is the level of Consequences.
Example: Severity to people is 4.
• Likelihood is the probability of Cause to occur.
Example: Likelihood is 3.
2) Residual Risk
• Severity level will be decreased due to the existence of Mitigation Safeguard. The Severity could be decrease one or two level depend on the amount of
Mitigation Safeguard.
Example: “Gas detection system” will soften the consequence by stopping more gas release and if ignited only small fire occur. The Severity could be
decrease one level.
• Likelihood level will be decreased due to the existence of Prevention Safeguard. The likelihood could be decrease one or two level depend on the
amount of Prevetion Safeguard.
Example: “Fully rated, PAHH and PSV” will prevent leakage or rupture, and then no gas will be relased. The Likelihood could be decrease two level.
3) Final Risk
• Severity level will be decreased due to the existence of Mitigation Recommendation. The Severity could be decrease one or two level depend on the
amount of Mitigation Recommendation.
Example: “Fire fighting system” will soften the fire impact or suppress the fire. The Severity could be decrease one level.
• Likelihood level will be decreased due to the existence of Prevention Recommendation. The likelihood could be decrease one or two level depend on the
amount of Prevetion Recommendation.
