A. “Journeying with the family to the End of life”

In this situation the patient Mr. W is a critically ill with multiple organ
failure, he is supported by machines to live and medicines to improve he’s
heartbeat. The nurse and the patient ha’s no verbal communication. The nurse
meet the family of the patient and have a conversation one of the nursing theory
that is used by the nurse is the Human to Human Relationship model because
the nurse talk to the family of the patient and the nurse maintain their hope and
avoid their hopelessness. The nurse observe that the family of the patient based
on their facial reactions they are very concerned and has a depth of anxiety they
had many questions about the patients conditions and the nurse answered and
explained well the present medical treatments and expected outcome of the plan
of care. This interaction of the nurse with the family of the patient is the
application of life perspective rhythm model because the nurse and the patients
relatives shares thoughts through language and the nurse focuses in enhancing
the developmental process of the patients health. When Mr. W passed away
three months later the nurse received a letter from one of the daughters of Mr.
W, the letters message is about thanking the nurse for the good care that her
father received. The nurse apply the human to human relationship model the
nurse take good care of the patient and the nurse has emotions in taking care of
his patient and treat the patient as a very important person, the family is
continuously involving in taking care of Mr. W and the family observe that the
nurse is taking good care of Mr. W and when he passed away one of the family
thanks the nurse because of that.

B. “ Revolving Door Syndrome”

In this situation the patient was being admitted several times in the hospital and
the nurse is handling the lady that fits the revolving door syndrome. For several
years the patient is in and out of the hospital the nurse felt exhausted all
knowledge, ability and skill she knew to help the patient because the patient is
verbalizing hopelessness, helplessness and despair also the patient refuse to
participate the activities for her to achieve wellness the nursing theory that can
apply in this is the interpersonal relations theory because the nurse purpose is to
help the patient identify their felt difficulties and that nurses should apply
principles of human relations to the problems that arise at all levels of experience.
also the Human to human relationship model is used in this situation one day the
patient is panic stricken, hyperventilating, restless and listless and the patients is
giving up to achieve her optimum health and wellness because her condition is not
getting better and she is afraid to die, the nurse comfort her and tell her the story
when she was a kid and her mother die and one time in her birthday the soul of her
mother visited her as a form of dragon fly and this story move the heart of her
patient and gives inspiration so the patients starts to be active and participates in
her therapy. The life perspective rhythm model is also used by the nurse because
the way that she convinces her patient is based on her own experience and gives
hop to the patient to never give up on life. Transcultural nursing is also used by the
nurse she apply the religion and belief of the patient to achieve wellness, the
patient has rosary and a crucifix beside her bed, believing in God is also one of the
factors that makes that patient recover.
