The form I-693, Report of Medical Examination and Vaccine Record

for Beneficiary, in a sealed envelope (please return the sealed
envelope)
This form will be prepared by our office and provided to the
beneficiary once supporting documents have been received. The
beneficiary will then need to take this form to the designated USCIS
civil surgeon for his/her medical examination.
