is a critical concept in
safety, as safe working
behaviors are the result
of first recognizing what
is dangerous. Of course,
hazardous energy

How to Safely Manage:
1. Every piece of power equipment
was chosen based on a set of
design parameters. One of those
parameters is that the fuel tank
utilizes a petcock to control
the flow of gasoline to the
carburetor. In support of that,
the carburetors also incorporate
up and drained of fuel for
extended periods of non-
operation. Finally, I keep the
amount of fuel to a bare mini-
mum so that if a fire was to
start, it would not get so out of
control due to the extreme rate
of energy release. In regards to
ignition sources, those items
were also chosen due to design
parameters or were placed up
high to ensure that any heavy
flammable vapors could not
reach the electrical contacts
(i.e. deep freeze’s compressor is
located at the top, and the air
compressor is elevated off the
floor).

• A coworker is working alongside the ship. As
you are walking by, you notice he is wearing a
full-body harness, but is not anchored, has
neither a self-inflating life preserver nor a
personal locater beacon in case he falls
overboard. You are with some others and no
one says anything, so your own conscious self-
justifies that everything will be fine. Besides,
he is a seasoned sailor; he must know what he
is doing. As you contemplate warning him, you
look back… and he’s gone.
• You are at a family gathering eating and
drinking, fun and games abound. The day
before, you turned down an opportunity to
take a CPR class that was being provided at
work. “No thanks, I have got to much work to
do…I will get around to it next time,” you
justified to a co-worker who was on their way
to the class. As the afternoon turns to
evening, kids playing all around, you suddenly
hear a helpless scream erupt from the table
where your father is sitting. Just as someone
yells “he’s having a heart attack,” your father
clinches his chest slumps out of his chair
unresponsive. No breathing. No pulse.
Everyone just stands there, panicking…frozen
with fear and the realization they do not know
what to do, let alone the courage to render any
form of effective first responder care.
• You are helping a coworker change the filter
on a fuel oil purifier. The fuel oil is being fed to
the ship’s engine and must be heated and
pressurized to extremely high levels. Just as
you are turning around, your coworker has by-
passed the steps required to shut down the
pump, depressurize the system, let the hot fuel
oil cool down and allow the purifier enough
time to stop spinning inside. He begins to open
up the canister. Even though he is wearing his
personal protective equipment, the exposure
to hot pressurized oil can still be extremely
dangerous; 2nd to 3rd degree burns that
permanently scar, inhaling oil mist into the
lungs causing chemical pneumonitis, literally
frying the delicate tissues of the eyes and the
potential for a fire to ignite in the engine room.

The examples above are based on
real events. They are placed at the
beginning of this article to draw your
attention and compel you to take a
deep breath just to realize the fragile
nature of your existence on this
planet. It is important to set the tone
of this article, as no happy ending
can be provided. All too often, I
see/read about people involved in
cases similar to the examples above.
Yet, sadly, people somehow justify in
their head that not taking action or
not actively caring for people was
acceptable. When someone is hurt
or killed, it is not an “accident” nor
is a year without a lost time injury
because of "luck."
In fact, in my experience as a safety
professional, it is both the human
response of taking action and
actively caring for people is what
defines whether a situation will
result in a safe or tragic outcome.
Through my 10-plus years as a safety
professional, many people have
inquired why I chose environmental,
health and safety (EHS) as my
career. Admittedly, for people to
understand my passion for safety,
I am challenged to adequately
sanitize the description of numerous
incidents that resulted in injury,
dismemberment or death that

head. Sadly, there are just too
many to count. I believe it is my
history — both professionally and
personally — of being saturated in
the ‘ugly side of safety’ that drives
me. From the many years of working
with my dad (new-home contractor
and cabinet shop owner), it was clear
to me from an early age that people
did not need to be hurt, electrocuted,
burned, dismembered or killed just
to provide a decent living for them-
selves or their families. After going
to university to study environmental,
health and safety at Pittsburg State
University (Kansas), it was there that
I found my calling. However, within
a few years working in the varied
industries of chemical, production
and government, I became aware
that to be a safety professional I had
to make the commitment to be a
safety leader. And, safety leaders

The blood pressure we normally
measure and refer to is the
pressure in midsize arteries, usually
the brachial (arm) artery. It is
measured in millimeters of mercury
(mmHg).
High blood pressure that is
untreated for many years can
cause irreversible damages in
many organs, including the brain,
heart, kidneys and blood vessels.
(See Table 1).
Understanding Blood Pressure
Readings
So what are those two numbers
given to you during a blood
pressure measurement, and what
do they mean?
The top number, systolic pressure,
is the peak pressure inside the
aorta (or any other artery), which
immediately follows the heart’s
pumping phase. The bottom number,
diastolic pressure, is the lowest
pressure inside the artery during
the relaxation phase of the heart.
Normal blood pressure is around
120/80 mmHg, but significant
individual variations do exist. For
instance, many people have blood
pressures around 100/60 mmHg and
experience no symptoms from this
“relatively low” reading. On the
other hand, for instance, someone
with an untreated, long-standing
hypertension of 170/95 mmHg
would feel lightheaded and weak if
his/her blood pressure dropped to
100/60 mmHg. It’s important to
note that blood pressure varies
constantly – even from beat to beat
– but more commonly with varying

medications, and illnesses
High blood pressure, known as
hypertension, is, by definition, blood
pressures of 140/90 mmHg or
higher. However, a single high
reading isn’t always cause for
alarm. Blood pressure should always
be measured under calm conditions.
At least three measurements under
similar, calm circumstances should
be completed in the evaluation of
high blood pressure. Like many
other parameters in medicine, the
trend over time is just as important
as the number itself.
What’s more, a significant propor-
tion of people have “white coat
hypertension,” which refers to
elevated blood pressure in the
doctor’s office due to anxiety. These
people do not have hypertension,
and the blood pressure measured
under calm and stress-free
circumstances will be normal
What Causes Hypertension?
The causes of hypertension are
unknown in greater than 90 percent
of cases. Some known causes
include renal artery narrowing
(which sends signals to regulatory
systems to elevate the blood
pressure in an attempt to increase
the blood flow), adrenal gland
tumors (which can produce certain
hormones that cause hypertension)
and certain drugs – especially
cocaine, amphetamine and related
compounds.
Individuals with certain risk
factors, also, are at greater risk

of developing hypertension.
Age, race and family history are
unmodifiable risk factors of
hypertension. The older we get,
the higher our chances are of
developing hypertension. African-
Americans are more prone to
developing hypertension, as are
those who smoke, participate in
excessive alcohol consumption
and salt intake, and those with
excessive weight. Physical
inactivity and elevated stress levels
can also cause or contribute to
hyper-tension. These modifiable
risk factors can be reduced, at
least to a degree, with lifestyle
changes. (See Table 2).
Signs, Symptoms and
Management of Hypertension
In a small percentage of people
with hypertension, headache is
a symptom; however, the most
common symptom of elevated
blood pressure is no symptom at
all. Thus, it can be a silent killer. In
other cases, people may present
with a stroke, heart failure or
kidney failure as a direct result
of long-standing, undiagnosed
hypertension. That’s why the
guidelines below from the
American Heart Association
can be helpful.

pump and supply blood to a
much larger body mass, thus
a higher pressure is necessary
to overcome the increased
resistance and propel the blood

body mass.
• Exercise Regularly
- When the body mass is reduced
by diet, exercise or both, the
pressure necessary to supply
the tissues with blood becomes
lower, thus the blood pressure
lowers accordingly.
- A regular exercise regimen will
condition the body and possibly
reduce body mass, contributing
to lowered blood pressure.
If the above measures fail or
you are unable to follow those
measures – or if the blood pressure
is significantly elevated –
medications may be necessary.
To determine appropriate drug
selection, a detailed dialogue with
your doctor is recommended.
And remember: the earlier the
hypertension is treated, the better
the chances are of preventing
complications.

The list of medical
events that have been
handled at sea is a long
one. Heart attack,
stroke, broken bones,
paralysis, and burns are
just a few of the many
conditions that have
been dealt with by
company vessels and
crew. The list can be
expanded to
dental conditions
such as an, abscess,
gum disease and tooth
decay. Even skin
conditions caused
by the elements and
of course the
occasional mystery
illness that comes
and goes.
Medical Person In Charge

So what should an individual do if
they feel ill or are in need of medical
attention while aboard the vessel?
The first step is to report the
situation to the Medical Person in
Charge (MEDPIC). Usually the
Master and the Chief Mate are the
MEDPIC giving primary care to
individuals in need. The Chief
Engineer and the First Engineer
have usually attended formal
medical training programs
as well. A MEDPIC is someone who
has attended a medical provider
program getting in-depth training
on subjects ranging from basic first
aid, CPR to severe trauma cases.
These training courses focus on
giving an individual knowledge on
how to manage the vast possibilities
of medical situations that may arise.
Quite often, the MEDPIC training
process involves working in the
Emergency Room or riding in an
ambulance with Emergency Medical
Technicians assisting them with a
wide range of medical and trauma
cases.
Even with the training Medical PIC
involved, there is no replacement
for consulting with a Doctor who is
assisted by a full staff of nurses and
practitioners. This is where Medical
Advisory Services (MAS) becomes
our biggest asset when dealing with
any scope of medical attention that
may arise at sea. As seen in recent
safety bulletin editions, MAS is
equipped with an on-call staff ready
for action 24 hours a day. During
my sea going career, I have been
assisted by MAS with cases
involving Flu epidemics aboard the
ship. MAS was a critical help with
two Helicopter MediVacs that
probably saved the individual‘s life.
They have given me advice on care
for upset stomachs, headaches, and
cold symptoms. More important,
when it comes to dispensing
medications, it is best that it is done
under the guidance of a Doctor and
MAS is the only Doctor available to
the ship.
In general, here is how the process
works aboard the ship. When a
member of the crew reports to the
Master or Chief Mate with an
ailment the individual is assessed to
establish the degree of attention
the individual may require. Most
cases are not emergency type of
events and the person is asked to
go to the ship‘s hospital where a
baseline of the individual’s situation
is established. Once the medical
facts and vitals are established, a
call to MAS is always prudent just
to ensure the correct care is being
provided and a case history is
established so that proper medical
care is advised promptly in the
event a patient's condition worsens.
In one particular case, a crew
member had come to me with chest
pain and reported shortness of
breath while walking up the deck
and feeling extremely fatigued. In
this particular case the ship‘s
operations were suspended and that
individual was given the highest
degree of attention. The Master and
Second Officer came to assist me
and MAS was called. MAS assisted
us in establishing a current and past
case history on the crew member
and vital signs were constantly
monitored with MAS providing
recommendations the whole time.
That crewmember was air lifted
from the vessel to the hospital.
The importance of disclosure of
preexisting conditions as soon as
that crew member signs aboard
can never be stressed enough,

up on the side table of
the CEO's office
collecting dust or as a
makeshift drip tray
liner for the coffee pot
in the employee break
room.
Not here at Chevron.
Medical Person In Charge
is one of the most important parts
of evaluating a patient’s condition.
Informing the Master of any
previous medical conditions and/or
prescriptions may very well be a
critical step in your care. The more
information the Master has in
advance will assist in the decision

because someone picks up a hard
copy somewhere, or it is forwarded
to them by email from an already-
existing subscriber. Since we do
not charge anything for being a
subscriber, anyone may forward
the Safety Bulletin.

in the Fleet and shore, I would like
