PETITION

PETITIONERS, through counsel and to this Honorable Court, respectfully state:

1. Petitioners are husband and wife, __ and ___ years old, respectively, Filipino

citizens, and residents in ______________;

2. Petitioners have been married on _________ and since then, have no legitimate

child or descendant and hereby desire to adopt jointly a minor, named JESHA FE _____, one (1)

year and seven (7) months old, legitimate child of___________ and __________, residents of

______________.

3. The natural parents of said child are still alive, they have____ children including

this child sought to be adopted, the father is a _________by occupation and the mother is a

housekeeper.

4. Likewise, said natural parents possessed full capacity to act, they were not

intemperate nor has they abandoned such minor, and with full knowledge of petitioners’ intention

expressly give their written consent to the adoption as shown by an instrument executed and

signed by them, a copy of which is attached to this Petition as Annex “A”, and made an integral

part hereof.

5. The custody and care of the minor, who is physically fit and in good health, have

been entrusted to the Petitioners since she was born onOctober 26, 2008 and the latter, since

then, has developed a kind of paternal love for the girl.

6. Petitioners possessed all the qualifications and none of the disqualification to

adopt said child, and are financially, healthily and morally capable to maintain, bring up, care for,

and educate said minor, properly and adequately.

 7. This adoption will serve the best interest and well being of the child.

PRAYER

WHEREFORE, Petitioners respectfully pray that, after due notice, publication, case study

of the child, her natural parents as well as the Petitioners, and report and recommendation of the

Department of Social Welfare and Development, and hearing, this Honorable Court, upon proper

disposition of the trial custody, issue a decree of adoption, stating, among others, that the child,

JESHAFE _____, be freed from all legal obligations of obedience and maintenance with respect

to her natural parents, and to all legal intents and purposes be the child of the Petitioners, and

that the surname of said child be changed to BUCASAS.

Other relief and remedies are prayed for.

2. The Petitioners and counsel have not commenced any action or proceeding
involving the same issue in the Supreme Court or Court of Appeals or other tribunal or agency; to
the best of their knowledge no such action or proceeding is pending the Supreme Court, Court of
Appeals or other tribunal or agency; there is no such action or proceeding which is either pending
or might have been terminated; should there be similar action or proceeding pending before the
