WHO : Mel John Icaro (Male) ID no. S-00071

WHEN : April 14, 2012 on or about 2230H

WHERE: Manufacturing 1 VF1 / AVF area

WHICH: A certain Mel John Icaro (Male) with ID no. S-00071 designated as Operator in
Manufacturing 1 VF 1 / AVF area was brought by his Section In-Charge Mr. Christopher
Ignacio to the Clinic due to lacerated wound approximately about half inch on his lower
knee right leg. He was attended and treated by our duty Nurse Ms. Luningning F.
Veniegas.

HOW: Accordingly, a blade of the cutter he is using fell down the floor while replacing diafoil at
the lower portion of the machine and the latter forgot to pick up said blade that has
dropped on the floor and accidentally, he kneeled on the floor unaware that the sharp
edge of the blade inserted openly on the floor resulting said injury.

ACTION TAKEN: The undersigned Internal duty Officer coordinated the matter to SO Cloyd S. Imbang,
Counterpart External duty Officer who requested to prepare company service vehicle to
bring subject employee to the nearest hospital as per advised by Mr. Carlo Bien Gonzaga,
Security Staff on duty per recommendation of attending Nurse (IPI) Ms. Luningning F.
Veniegas. Soon after at about 0038H midnight, subject employee was brought to
Hospital on board company vehicle bearing plate no. XPF – 417 driven by Mr. Rene Boy
Ofrin escorted by SG Onte Darius.

MEASURES / RECOMMENDATIONS:

 To reiterate to all employees to always be careful while in the performance of work.

They have to always think for safety first during and after work on their respective
work stations.
 To secure unattended items particularly sharp pointed objects to avoid further injury
or same incident in the future.
