As part of continuing MDC Safety and Health Programs, we would like to inform you that we will conduct a wellness
program such as jogging, zumba workout, basketball, badminton, table tennis, volleyball and shoreline
cleaning. Otherwise, workers who are unable to participate should have a valid reason and are obliged
prioritize their work.

Program Purpose:

1. to promote health awareness

2. to improve employees fitness and health
3. to help assess the necessity for weight loss or the prevention of further weight gain.
4. to encourage the employees in engaging/changing their lifestyle that support health body weight.
5. to determine the most appropriate dietary recommendation for overweight and obese employees.

This will be scheduled every Saturday from 7:00 to 8:00 am. The venue will be held mostly inside the MDC premises
or at the El Nido Cove. I am requesting for all MDC employees especially the overweight and obese workers to
participate for this program.

After the activity all workers are require to wear MDC uniform and return to their respective work. Any
workers not complying with the above mentioned activity and instruction will undergo proper sanction
and admonition according to MDC Code of Discipline and MDC EHS rules and regulation.

Listed below are the offenses to be imposed:

Offenses: First Offense – Verbal Warning

 Second Offense – Written Warning
Third Offense – 3 Days Suspension
Fourth Offense – 5 Days Suspension
