AFFIDAVIT OF ABANDONMENT

I, xxxxxxxxxxx nee xxxxxxxxxxxxxxxxxx, Filipino, of legal age, a

xxxxxxxxxxxxxxxxxxx, married but now separated de facto with my husband
xxxxxxxxxxxxxxxxxx, with official address at xxxxxxxxxxxx,
xxxxxxxxxxxxxxxxxxxxxxx, Philippines, after having sworn in accordance with
law, do hereby depose and state:

1. That I was married to xxxxxxxxxxxxxxxxxx on February 5, 2004 at

xxxxxxxxxxxx xxxxxxxxxxxxx;
2. That out of the relationship were born my children with the following
circumstances as follows:

mistress. Furthermore, I learned that my husband has already a child with his
mistress.
4. That sometime on June 14, 2014, I could no longer take the constant
conflicts and quarrels with my husband for the above-mentioned reason that
I decided to transfer my residence to an apartment in xxxxxxxxxxxxxxxxx;
5. That I took with me my four children, namely xxxxxxxxxxxxxxxxxxxx;
6. That since then, my husband and I have been living separately;
7. That since that time that I left him, he also stopped supporting me and our
children that I had to take care of the four from that time until now;
8. That since that time also, my husband did not give any financial or monetary
support for the children’s food, medicine, and education up to now;
9. That in fact, I have not talked nor communicated with him for he had
abandoned us already. From that time, he had not extended any financial
support whatsoever;
10. That I am executing this affidavit to attest to the truth of the foregoing
facts, particularly that of the abandonment of my spouse, and for whatever
legal purpose this affidavit may deem serve.
