AFFIDAVIT

I, R. JAYARAJ, S/o R. Rathnam Raju, residing at : Sirwar Tayamma

Camp, Tq-manvi, Dist-Raichur, Karnataka- 584123. do hereby solemnly
affirm and state on oath as follows:
I hereby declare and state that, myself being registered owner of the Two
Wheeler Vehicle Pulsar 150CC, bearing Reg No. KA-36 ER-8059,
chassis No. MD2A11CY2JWF38705, Engine No. DHYWJF54151,
Model 2018, with up to date documentation records.

Further I hereby declare that due to my personal work I am not able to

look after and maintain the said vehicle personally. Hence, I hereby
authorizing and delivered the said vehicle to Mr. SALA NAGAVENKATA
PRASAD, S/o S Umachandrashekar, residing at : W No.1, Sriramnagar,
Gangavathi, Koppala, KA-583282.

To maintain and look after the said vehicle and attach the said vehicle
with any Travels Company on my behalf and thus he can be liable to
collect for which I have no objection to him in whatsoever manner over
the above declaration.

In This Consideration if anybody raises any disputes, in due course

I shall be alone held responsible.

I am submitting this affidavit before concerned authorities over the above

declaration.
