Attached here are the details and agreement as follows;

1. The client (Student) will have our tutorial services 3 times a week.
(Tuesdays, Thursdays and Saturdays)

2. Tutorial fee is Php 250 per hour, and should be payable on a

weekly basis. And an additional fee of Php 250 (per hour) for any
extended time.

3. Deposit is non-refundable but can be consumable by the student.

4. Reading and other source materials are provided by the teacher,

but photocopies of worksheets for student should be provided by
parents.

Attached in this contract are the terms and conditions, as well as the coverage
and agreement of the Center and client.

1. COVERAGE: GUIDING STAR shall provide tutorial services to one (1) student
herein represented by the Client for a weekly fee of Seven Hundred Fifty Pesos
(Php 750.00), starting January 12 – 19, 2019. With an advance deposit of Seven
Hundred Fifty Pesos (Php 750.00), non-refundable but can be consumable by
client. The tutorial shall cover the following subjects: Commented [JCM2]: Can you pls enumerate.
_____________________________________ and shall be rendered every Saturday,
Tuesday and Thursday from 11:00 am – 12:00 pm. The client opted for the
Home-Based tutorial.

2. TERMS OF PAYMENT: Tutorial fees must be paid in advance on a weekly or

monthly basis, depending on the agreement of both the teacher and the
client. As a rule, tutorials shall only commence/continue upon payment of fees,
on or before the first day of each week/month starting January 12, 2019.

Additional fee of Php 250.00 per hour is required for any extended time of
tutorial. Payments can be made only through any BPI branches nationwide.
The client must keep the deposit slips. It will serve as his/her temporary receipt.

3. PRE-TERMINATION BY CLIENT: Should the client decide to discontinue with the

program, he/she should do so by giving written notice to GUIDING STAR at least
seven (7) days before the intended date of termination. Clients are however
obliged to pay the full amount of the program. Commented [JCM3]:

4. VENUE: Tutorial services may be conducted at the Center or at client’s to what program?

expenses of the tutor directly related to the tutorial including transportation

5. RESOURCES: Reading and other source materials are provided by the Center.
But photocopy expenses of worksheets and additional materials shall be
provided by the client/student.

6. NO REPRESENTATIONS: GUIDING STAR does not make any representation,

guarantee, or warranty of any kind whatsoever that the student shall pass or
excel in any and all examinations or tests or shall gain mastery in the subjects
concerned. GUIDING STAR and its teacher/s shall nevertheless exert its best
efforts so that the Client shall benefit from all the fees he/she has paid to the
Center.

7. CONFIDENTIAL MATTERS: For the client’s protection, GUIDING STAR does not
allow its tutors to divulge, during or after this contract, any information
concerning any of GUIDING STAR’s clients. The center asks the same from its
clients. Likewise, we request CLIENT not to discuss any and all terms of this
contract, with anyone especially our tutors.

IMPORTANT REMINDERS

1. ALL CONCERNS MUST BE DIRECTED TO THE CENTER: Changes in schedule,

frequency, and length of the tutorial services must be arranged through
contacting GUIDING STAR.

2. IF A STUDENT IS ABSENT FROM A TUTORIAL SESSION, he/she can make up for

the lost time upon the approval and arrangement with the Center. Cancellation
of tutorial session should be made at least 24 hours before the scheduled
session, otherwise, no make-up session will be given.

2.1. IF A TEACHER IS ABSENT FROM A TUTORIAL SESSION, he/she can

make up for lost time and shall be divided per session depending on
the hours of absences.

3. THE CENTER AIMS TO PLEASE ITS CLIENTS. Complaints, questions, or

suggestions are most welcome and must be directed to the Center as soon as
possible.

This contract is legally binding and final. The signatories acknowledge that they
enter it with their own free will and it is not reversible.

This contract shall be governed by the laws of the Philippines.
