Learning Feedback Diary

Today was our delivery room orientation. We were taught about the setup of the

instruments, tools, and the medications. The rules and policies were explained for us to

be guided and to abide to avoid possible incidents. I learned that the hospital is

practicing the “unang yakap” but not all the ideal principles are always observed. But

they always apply the aseptic technique for the patient’s safety and for the medical team

also.

The head nurse introduced as to the nurses on duty and the physicians that work

here at the DR.

Learning Feedback Diary

Today I was unfortunate to not having a case but I got the chance to observe an

actual delivery and the procedures performed. The return demonstration and the actual

scenario is not really different from each other but there are really instances that they

differ on situations. I observed that the nurses can adapt to different situations and

maintaining the aseptic technique at the same time.

I also had a hard time catching up with the endorsement but I know I will improve

through time and experience.

Learning Feedback Diary

Today as usual, had a hard time catching up with the endorsement. I also had

three cases during our shift, I was given the task of handle delivery, assist delivery and

cord care. I learned to do these procedure on an actual setting and somehow it boosted

my confidence. My first case was assisting delivery, I helped the doctor and nurses by

handling them the equipment and cleaning the table after the delivery. My second case

was cord care and I learned to take care of the baby after cutting the cord, administering

vitamin K and hepatitis B vaccine and terramycin. My last case was handle and I

learned to catch a baby, deliver the placenta and remove left particles in the uterus.

Learning Feedback Diary

Today I was given the task of assist delivery, and it was the only case I got today.

I was able to perform fundal push and assisted Dr. Valera in performing episiorrhapy.

After the delivery, I cleaned the used instruments and the bloody delivery table. Before

leaving, Dr. Valera gave us a fast lecture about partograph to be applied for use in labor

watching. So far, our night shift duty in the delivery room was fine and a good learning

experience.

Learning Feedback Diary

Today is our last day in our delivery room rotation. Personally I am sad because

our DR exposure was quite short but I am still glad ad thankful that our RLE time was

worth it. Even in a short period of time we were able to experience almost all the things

that are necessary for our own growth and to boost our confidence in the field. During

our exposure I realized that we really need to adapt in every situation and to utilize all

available resources for the benefit of the patient.

I am very thankful to our clinical instructor that guided us all the way and putting

her faith in us without doubting our current skills. I still have long way to go and I hope to

do good in every rotation.

(Delivery Room)

Goal:

After a 24hr duty at the DR, I will enhance my K, A, S in the care of client who will
deliver via NSVD

Objectives:

After the 8hr shift at the DR, I will provide quality nursing care to my assigned
patient through:

I. PROVISION OF SAFE AND QUALITY CARE

1. Obtain obstetrical history including parity, gravida score, LMP, EDC,


AOG, BOW, onset of true labor.
2. Checking vital signs.
3. Conducting physical examination.
4. Perform Leopold’s maneuver.
5. Checking fetal heart rate and fundic height.
6. Monitor progress of labor/uterine contractions as to:
 Frequency
 Duration
 Intensity
 Interval
7. Observe timely rupture of membrane.
8. Coach mother on process of labor.

Performs Function during Actual Labor

1. Transporting client safely while providing privacy.


2. Assist mother in laboratory in litothomy position.
3. Give perineal care using sterile technique.
4. Perform proper hand scrub.
5. Wears gown and gloves accordingly to hospital policy.
6. Perform Ritgen’s maneuver safely.
7. Coach mother on breathing and pushing techniques.
8. Delivers baby and placenta carefully.
 Checks and manages cord recoil correctly.
 Clamps and cuts the cord correctly.
 Identify signs of placental separation.
 Checks the characteristics/completeness of the placenta.
9. Assesses amount of blood loss.
10. Employs interventions to achieve and maintain a well contracted uterus
to prevent/control hemorrhage.
 Uterine massage
 Correct administration of oxytocin
 Cold compress
11. Assesses presence and degree of laceration.
12. Assist in episiorrhapy.
13. Check size, consistency, and location of uterus.
14. Perform perineal care and applies pad correctly.
15. Provide emotional support to the mother throughout labor and delivery.
