AFFIDAVIT OF DESISTANCE

I, ERICA PANTA, of legal age, single, residing at Prudence Street,

Muntinlupa City, after being duly sworn in accordance with law, hereby depose
and state:

1. I am the Complainant in the above-captioned case docketed as

______________ before the Office of the City Prosecutor of Pasay City.

2. After a careful deliberation of the facts and the law involved in the case, I
found that the initiation of the criminal action, through my affidavit were
all based on a grave misapprehension of the facts and circumstances. In
truth, the same was merely a result of a simple misunderstanding.
Moreover, the damages sustained by the vehicle I was driving was purely
accidental and that no person is at fault.

3. For these reasons, I am no longer interested in pursuing this criminal action

and shall desist from further prosecuting the case involving the vehicular
accident that transpired on 5 January 2019 in Pasay City.

4. I am executing this Affidavit to attest to the truth of the foregoing, of my

own free will, free from any undue influence or coercion.
