Introduction including overview of area inspected and activities taking place
On 22/02/19 the 3*660 MW NUPPL Ghatampur thermal power project has been located
at vill- Lahurimau kasimpur, post- Samuhi, tahsil- Ghatampur, District- Kanpur nagar-
208001 has inspected at the time 10:00 AM to 11:30 AM to find out the health and
safety management system of the site. The purpose of this observation to identified the
hazard of the workplace activities.
The construction works were carried out at the site and approx 1100 workers are
working in one shift. Day and night both shifts are used for the carried out the works, the
digital attendance system are use to maintain the attendance of the workers and also a
register maintain by the guard for in and out system. The welfare facility of the worker’s
are very good such as neat and cleaned washroom first aid station almost every site,
the equipments used by the workers at construction site are Forklift, crane, grinding
machine, man lifting, welding machine, metal cutter
From that the major hazard have identified and made recommendations to avoid the
personal injury to the workers as well as property damage, because a minor problem
can create lots of hazard for the site, which is not cared by the workers. As per the
statement of the newspaper and the finding of the goggle, the net worth of company are
approx 1700 cr. And the company achieved standards are ISO 14001:2004

Executive Summary
NUPPL Ghatampur thermal power project at Lahurimau kasimpur, post- samuhi, tahsil-
ghatampur, district- Kanpur nagar-208001 has been inspected for the health and safety
measurement , it was found some good observation as well as some critical
observation, which should be improve as earlier as possible they were:-
It is observed that at ESP area the concrete mixture vehicle driver was stand under the
boom without safety helmet, Safety goggle & reflective jacket, it may be caused fatal ,
fracture, eye injury , head injury , that means the vehicles driver are not aware with the
health and safety responsibility. Management should provide proper training to the
vehicles driver to avoid such major problems; if it reoccurred then disciplinary action
must be taken.
It was observed that, at zone S6, deep open excavation has been found without any
barricading at CW pipe line area. These hazards are irresponsible performance of the
workers, it may cause fall of material, persons, animals or vehicles, which can create
the major accident like fatal, fracture, head injury. To control this irresponsibility activity,
provide the hard barriers, signs and warning light for the prevention from unauthorized
persons entering the site or provide proper training on excavation process by which they
can growth their awareness.
It was observed that, at the zone A6 , there are two person working the hot work of
welding and one empty fuel can kept near the wall, where hot work was carried out ,
which can cause of explosion of fire, To control this hazard , stop working and kept fuel
cane on their proper place , also proper supervision given to workers, how to deal with
hot work.
It was observed that, at the vehicle working station there are no separation pathways of
pedestrians’ or vehicles, All are using the same path for moving and also no any sign
board are using at the site, which can cause collapse of vehicles and pedestrians’, To
control this hazard, management should provide temporary barricading by segregating
pathways immediately as well as provide permanent barricading between the pathways
of pedestrians or vehicles.

It was observed that, at the zone S12, there is a work on height is carried out, so
scaffold is used for their working platform but it seems that a worker are trying to

any cut injury in their hand , To control this hazard management should provide the
proper training for how to work on height and provide the hand gloves for performing
their task safely.
The senior management or the director of the company is the accountability person of
the company, his responsibility to provide the safe workplace or safe environment to
carrying out the task safely. The workers faces many problems in their in their daily
activities for earning money for their family and accidents are not the cost paid to the
workers for their daily works. So the senior management take cares of his workers for
performing task in safe manner. If any accident occurs then there are many legal
problem faces by the management or also the money are wasted to fulfil the problems
of incidents, So all the hazard which is discussed above can cause of the major
problems for the management, which should be improved

Main Findings
1. Operating concrete mixture vehicle without using safety helmet, safety

goggles or reflective jacket.
At the time of operating the concrete mixture vehicles , the driver should must
wear the safety helmet , safety shoes , safety goggles , reflective jacket, all the
essential PPE which is used in that process, if it is not followed then any serious
or major accident can happen, at the time of observation the driver are not using
the safety helmet , safety goggles or reflective jacket, which is very
irresponsibility performance by the driver, which can cause serious accidents if
something falling on their head or some ejected particles enter in their eyes.
Recommendations:
a) It is recommended that provide the proper training to all drivers for the
awareness of health and safety responsibility- INR 30000
b) Higher some competent person for monitoring the work activities- INR 25000
Legal Requirements:
a) Employers have duties concerning the provision and use of personal

protective equipment at work.
b) The personal protective equipment regulations 2002 and the personal

protective equipment at work regulation 1992(as amended) give the main
requirement
2. Excavation has been found without any barricading at CW pipe line area.
All the deep open Excavation should be properly barricaded by hard barricading
System by which no any incidents such as fall of persons, materials, animals, or
vehicles occur, or also proper sign of excavation should be placed at their place,
by which any one can aware with the hazard, at the CW pipe line area it is found

problems for the workers as well as the management.
Recommendations:
a) It is recommended that provide hard barricading from all side of the

excavation- INR 20000
b) Provide warning lights or proper sign board for alerting the high risk- INR

c) Provide proper training to all the workers about the Excavation process- INR

Legal Requirements:
a) It is legally important that all the Excavation are must be barricaded or proper
warning light or sign board are used at that place
b) When Excavation work is being carried out adjacent to an area where the
public or worker who isn’t usually engaged in the work may pass, an
employer must have ensure that the excavation is adequately guarded by a
fence , guardrail or covering sufficient to prevent a person from falling into the
excavation. OSHA [sec. 26.12(1)].
c) Excavation must be guarded by effective railings or barriers to prevent

workers from falling into excavation. OSHA [sec.410(1)].
3. Hot work was carried out in the workplace, where an empty fuel cane kept
inside the wall.
It is important that all the hot work of workplace will carry in safe way, all the
combustible material should be kept away from that area or those material which
is not able to keep away should be properly covered , at the zone A6, it is

empty cane kept near the wall, which can create fire or explosion.
Recommendations:
a) Provide training to the workers, how to deal with hot works- INR 10000
b) Provide suitable storage area to keep the fuel cane on the proper place- INR

a) Flammable, combustible, or ignitable materials should be kept a minimum of

20 to 35 feet away from the hot work
b) It is important to have a permit signed to show that all phases of the work has

4. Not barricading between the pathways of pedestrians or vehicles.
It is important that pathways of pedestrians’ or vehicles should be segregated

with the hard barricading system by which not any incident of collapse occur
between the pedestrians or vehicles, proper sign board should be placed on their
position for alerting the person, At the vehicle working station, It is observed that
no any segregation of pathway found, pedestrians’ or vehicles both are using the
same pathway, which can create the collapse between them.
Recommendations:
a) It is recommended that provide hard barricading between the pathways of

pedestrians’ or vehicles- INR 50000
b) Provide suitable sign board at their place to alert the person- INR 20000

cause the fall incident, at zone S12 a worker working on height and try to
overreaching in their work, which can cause the major accident like fatal or
fracture.
Recommendations:
a) It is recommended to management for providing the training to the workers about

work on height – INR 20000
b) Higher competent person for monitoring the activities – INR 20000

Conclusion
1. The safety inspection was carried out at 3*660 MW NUPPL Ghatampur thermal
power project has been located at vill- Lahurimau kasimpur, post- Samuhi, tahsil-
Ghatampur, District- Kanpur nagar-208001, the inspection shows some good
practice such as some works carried out, where the bricks rails are not present but
the workers are using hard barricading with barricading red net, which save the
persons or object falling from height and also motivate the workers for feeling safe
or eye wash station which is placed near the work station, which is very good for
the eye of the workers but there is also some critical observation found such as hot
work near the flammable fuel cane, pathways without segregating of the
pedestrians’ or vehicles, which can cause the major problems for the workers as
well as the management
There is some point based on the finding, which should be improved, it should be
considered or implemented with immediate effect:-
 Provide suitable and sufficient storage area for the storage of flammable
material
 Provide proper hard barricading between the pathways of pedestrians’ or
vehicles
 Provide warning lights or proper sign board for alerting the high risk
 Provide proper training to work on height
 Instruct the Japanese technique 5s approach for effective housekeeping
 Provide hard barricading system at the CW pipe line area, where open
excavation found.
 Higher some competent person for monitoring the activities
 Provide proper lightening in the access area of the maintenance
department
 Provide proper rest area for taking rest of the workers

 Provide proper training on the hot work
All the recommendation which is indicating the poor safety culture ,it should be
improved immediately as soon as possible, The senior management or the director of
the company is the accountability person of the company, his responsibility to provide
the safe workplace or safe environment to carrying out the task safely. The workers
faces many problems in their in their daily activities for earning money for their family
and accidents are not the cost paid to the workers for their daily works. So the senior
management take cares of his workers for performing task in safe manner. If any
accident occurs then there are many legal problem faces by the management or also
the money are wasted to fulfil the problems of incidents, so all the hazard which is
discussed above can cause of the major problems for the management, which should
be improved
