I. Project Summary
According to the world health organization’s assessment of factors responsible for global burden
of disease; tobacco, alcohol and illicit drugs contributed 12.4% of all worldwide deaths in the
year 2000. “The harmful use of alcohol alone, results in 3.3 million deaths worldwide” each
year. The negative consequence of substance abuse is not only limited to death. Substance abuse
or the harmful use of psychoactive substance leads to dependence syndrome; “a cluster of
behavioral, cognitive, and physiological phenomena that develop after repeated substance use
and that typically include a strong desire to take the drug, difficulties in controlling its use,
persisting in its use despite harmful consequences.” Substance abuse also leads to various social,
economic and health related problems. Among the problems; criminal activity, domestic
violence, child abuse and neglect, physical and psychological illness, reduced productivity, loss
of employment, homelessness, and infectious diseases are some. Despite all these harmful
impact on human life and sustainable global development, for far too long substance abuse has
not been given enough attention. However, the recent stride by the United Nations General
Assembly to adopt the prevention and treatment of substance abuse as its integral part of
sustainable development agenda to transform the world by 2030 is a step forward in the right
direction. Under target 3.4 and 3.5 of goal 2030 nations are asked “by 2030 to reduce by one
third premature mortality from non-communicable diseases through prevention, treatment and
promotion of mental health and wellbeing” and to “strengthen the prevention and treatment of
substance abuse, including narcotic drug abuse and harmful use of alcohol.”
In Ethiopia the prevalent use of harmful substances is a growing phenomenon and the most
frequently used substances are Alcohol and Khat followed by cannabis and solvents. Although
studies are scant and less extensive to measure the growth rate and impact of the problem, the
available studies show that substance use is considerably rising especially among at risk
population which includes the youth and street children. According to a study by Abebaw
Fekadu, Atalay Alem, and Charlotte Hanlon of Addis Ababa University and King’s College
London, in the year 2007 the prevalence of hazardous drinking was at 3% meanwhile the
prevalence of khat use varied from 0.3% to 64%.

The harmful impact of substance abuse in Ethiopia also needs to be studied. However, the few
studies done on the subject indicate an increased prevalence of suicide attempts among alcohol
and khat users (Alem et Al., 1999a) Physical illness, injuries, mental distress, sleep disorders
(Belew et. Al., 2000) increase in HIV risk factor among khat users (Abebe et Al., 2005) and
showing up late, leaving early from work or regularly missing work was reported among
University staff who used khat (Gelaw & Haile Amlak, 2004). In addition, according to the
world health organization non communicable disease accounts for 34% of annual death in the
country this according to the ministry of health could have been prevented by avoiding
behavioral risk factors such as tobacco use and the harmful use of alcohol. In addition, to the
various health and social problems substance abuse has also huge impact on productivity and
employment which in turn affects the economy in general. Therefore, substance abuse can be
potential sources of risk on sustainable development. Unfortunately, so far there has been limited
response by the government and nongovernmental organizations alike. There are no adequate
policies to influence the levels and patterns of substance use. In addition, treatment and
prevention are low priorities in the health care system. For instance, there are only limited
numbers of medical centers that give substance abuse treatment in the country. These include
three public hospitals in Addis Ababa; namely St. Paul, Emanuel and Zewditu hospitals with a
capacity of 5, 14 and…. beds respectively that are dedicated to substance abuse patients. In the
last twelve years, these three hospitals have only treated 1427 inpatients of substance abuse.

Therefore, based on the risk of substance abuse and the limited response, this project is crafted to
help contribute toward the solution by involving different stakeholders in the prevention and
treatment of substance abuse through internationally accepted methods that has been proven to
work. In addition, the full implementation of the project will help to reduce the risk for
sustainable development. The project will also supplement the Growth and Transformation plan
of Ethiopia by reducing the associated risks of substance abuse on the transformation and growth
of the country while helping to achieve the United Nation’s 2030 goals.

The project will be implemented in collaboration with the ministry of health, St. Paul
Millennium Medical Hospital, and other governmental and private stakeholders. While St. Paul
Millennium Medical Hospital provides technical and other supports to New Life organization the
Ministry of health will regularly follow up on the project operation while leading the evaluation

of the project. New life Charitable Organization will also work with other community based
organizations and the general public for the successful implementation of the project.

II. Organizational Background and Capacity to implement the project


New Life Charitable Organization is a newly registered and certified organization by the Federal
Charities and Societies Agency of Ethiopia. The organization is established in 2016 to provide
treatment for victims of substance abuse and preventive service for at risk segment of the
population in the city of Addis Ababa. Its Founders are a group of women medical professionals
from St Paul Hospital Millennium Medical College with a combined experience of over 25 years
in substance abuse and other medical units. Its director, Nurse Yirgedu Habtu is a distinguished
nurse from St. Paul Hospital who has been one of the recipients of governmental awards of
recognition in 2015 for outstanding performance. In addition, Nurse Yirgedu Habtu is a devoted
mother of four who has seen the impact of substance abuse firsthand both as a private citizen
with family members who are victims of substance abuse and a staff at St Paul Hospital
Millennium Medical College’s substance abuse unit. Among other members of the organization
fully recovered and recovering substance abuse victims are included. Therefore, the diversity in
experience and knowledge offers the organization a unique set of opportunity and capability to
implement the project successfully.

III. Vision, Mission and Objectives


Vision: - To see substance influence free society for prosperous Ethiopia
Mission: - Our mission is to deliver result oriented prevention and treatment of substance abuse
through internationally accepted methods and the latest technology in the health care industry
with unparalleled service to our beneficiaries by creating an atmosphere of learning and a culture
of excellence while leading the way in developing new treatment and prevention methods
through research and development.
Development Objectives
 Contribute toward the ministry’s goal of improving health status of the population by
providing prevention service for at risk segment of the population and by providing
treatment for victims of substance abuse.

 Contribute toward the ministry’s goal of improving health emergency risk management
by gathering, analyzing and providing data on the issue of substance abuse
 Assist the ministry of health and other governmental agencies to develop guidelines and
policies on the issue of substance abuse.
 Help achieve the United Nation goal “by 2030 to reduce by one third premature mortality
from non-communicable diseases through prevention, treatment and promotion of mental
health and wellbeing” and “strengthen the prevention and treatment of substance abuse,
including narcotic drug abuse and harmful use of alcohol” in Ethiopia.
 Develop further prevention and treatment methods that work and can be replicated
throughout Ethiopia and beyond through research and development

Immediate objectives
 Increased public awareness about substance abuse
 Assist other community based organizations such as schools in the prevention of
substance abuse
 Increased support and health care access for early intervention and treatment of substance
abuse victims and their families
 Provide relapse prevention support for patients that underwent through rehabilitation
 Provide life changing rehabilitation and reintegration activities and trainings to help place
victims back on a productive path.

IV. Target group/beneficiaries


Direct beneficiaries
 360 long term rehabilitation patients and their families
 500 outpatients and their families
Indirect beneficiaries
 The communities of 3 Woredas around the rehab center
 High school students within the determined Woredas
 College and University students within the determined Woredas
 The Woreda adminstration, police, and other community based organizations and
institutions
V. Implementation plan and Time-frame

The project will be implemented in two phases; during the first phase, New Life Charity
Organization envisages at least to reduce the cumulative effects of substance abuse by
establishing rehabilitation center in Addis Ababa that provide treatment for in and out patients
while performing prevention activities as well.
During the second phase, the center will own 30,000 square meter land to construct a multi
purpose rehabilitation center for both male and female patients. Furthermore New Life Charity
organization will expand to regional cities and establish additional treatment and rehabilitation
centers while conducting research and development activities in order to enhance the prevention
and treatment of substance abuse.

Phase I
1. The organization will set up a rehabilitation center with a capacity of treating 360 male
in-patients and another 860 out-patients of both genders within a year form
commencement
2. Develop operational manuals pertaining to all functions of the organization with in six
months from commencement
3. Develop treatment plans and methods that are specific to Ethiopia’s context. Within a
year from commencement
4. Develop prevention strategies and plans that are specific to target groups within a year
from commencement
5. Begun Rehabilitative treatment and early intervention for beneficiaries within a year from
commencement
6. Conduct prevention workshops and trainings in schools, universities and other
community based organizations starting from the second year of implementation
7. Begun implementing sustainability strategies with in the first year of commencement.
8. Begun collecting, analyzing and storing data on substance abuse and related problems as
soon as treatment and prevention activities begun
9. Periodically evaluate the success of treatment and prevention
10. Prepare activity and financial plans for the second phase of the project and begun
preparations.

Phase II

The second phase of the project will start at the end of the first project duration of three years
and New Life Charity Organization will implement the following major activities.
 Building a facility to accommodate both male and female patients.
 Begun providing detoxification and other services
 Begun research and development activities with the collaboration of other community
based organizations.
 Actively engage in assisting policy makers in the development of policies pertaining to
substance abuse.
 Facilitate the replication of project benefits to other parts of the country.

VI. Description of project Activities


Prevention
Research shows prevention is more effective and less costly than treatment, therefore New Life
Charity organization will perform the following activities with the aim of enhancing prevention
mechanisms that can be replicated throughout the country.
1. Provide awareness creation workshops and trainings: - In collaboration of community
based organizations, faith based organizations, Kebele, Woreda, sub-city offices; New
Life Charity Organization will provide the following activities.
 Provide prevention training based on personal and social skills and social
influence
 Provide parenting skills trainings to help parents become better parents
 Empowering Mothers
2. Capacity building to schools: - Assist schools especially universities in addressing the
issue of substance abuse through school policies and cultures.
3. Bring together different actors in the community to address substance abuse
4. Media Campaign to have an effect on public perception about substance Abuse

Treatment – New Life Charity Organization will perform the following activities for early
intervention and treatment of substance abuse.

1. Screening Activity: - Through this activity the center will assess the severity of
substance use and treatment needed.

2. Brief intervention: - Increasing a person’s awareness of substance use and encouraging


changes in behavior.
3. Refer patients to partner hospitals for detoxification: - If patient needs extensive
treatment that cannot be provided by the center, the center will refer the case to its partner
hospitals.
4. Receive referral from partner hospitals for rehabilitation: - New Life charity
organization will not provide detoxification service during the first phase of the project
implementation. Therefore, the center will depend on its’ partners especially St. Paul
Millennium Hospital for the service and will only accept patients that underwent through
detoxification and are referred by the partner hospitals for further rehabilitation. The
center will also require referral letter from the concerned government office in regards to
the patients’ inability to pay.
5. Admit Patients based on pre-defined criteria: - New Life Charitable Organization
admits patients based on pre-defined criteria.
6. Once admitted patients will receive the following medical and therapeutic care:-
6.1 Intensive rehabilitation treatment based on the specific condition of the patient.
6.2 Substitution medications to relieve drug craving.
6.3 Group and individual counseling and therapy sessions.
6.4 Guidance and support for behavioral changes; and participation in peer help
groups.
7. Provide outpatient follow-up and relapse prevention for patients who undergo the
rehabilitation process
8. To further boost recovery and reintegration, the center will provide the following
support activities and programs
8.1 Life skill training
8.2 Personal financial management training
8.3 Job readiness training
8.4 Nutrition training
8.5 Indoor and outdoor games
8.6 Sports and gym
8.7 Gardening and other outdoor activities

9. Provide meal for inpatient beneficiaries

Admission Criteria for long term rehabilitation patients - The following criteria shall
apply only to the first phase of the project and will be amended during the second phase of the
project.
1. Patient needs to go through detoxification process before being admitted to the center
2. Patient needs to be referred by partner hospitals.
3. Patient needs to bring a letter conforming inability to pay from keble or local
governmental offices.
4. During the first phase of implementation New Life will accept only male patients due to
capacity constraint.
5. The patient must give their consent to the treatment and shall agree to commit themselves
for the full duration of the prescribed treatment.
6. The patient must agree to the rule of conduct of the center
7. For early intervention treatments beneficiaries must bring referral letters from one of the
following institutions. Local governmental offices, community based or faith based
organizations.
8. New Life Charity organization shall not refuse treatment based on ethnicity or religion of
the beneficiary

VII. Outputs and Expected Outcomes


Project Outputs
Phase I
 Treatment plans and methods specifically suited to Ethiopia’s context are developed
and/or adopted.
 Prevention strategies and plans that are specific to target groups are developed and/or
adopted.
 New Life rehabilitation center is set-up according to specification
 Several project partners and stakeholders engaged in the prevention and treatment of

Phase II

Project Outcomes – During the first phase of project implementation New Life Charity
organization expects the following outcomes.

 360 victims of substance abuse are treated with success rate of 50%. The success
rate will be measured based on completion of the program and sobriety rates after
six months from treatment.
 500 out-patients that include early intervention and relapse treatment patients are
treated with success rate of 70%
 More than 360 families are trained to properly deal with family member/s with
substance use disorder.
 More than 500 parents are empowered with parenting skills that would help them
become better parents.
 Increased awareness in the surrounding communities within 3 Woredas adjacent
to the rehabilitation center

VIII. Organization and management


The project holder, New Life Charity organization, has a general assembly of 20 members,
representing the membership. The Board of directors is the highest sovereign body that provides
strategic direction to the organization. The Executive Director, who is accountable to the Board
of directors, is responsible for managing the day-to-day activities of the Organization. The
Executive Director is also member of the board and reports to the Board.

Under the Executive Director’s guidance and leadership, the project coordinator is
responsible for the overall project activities.

IX. Plan to Ensure Community Participation


New Life Charity Organization believes the project will not be effective without the full
cooperation and participation of the community at large. Therefore, the organization will seek to
forge a closer relationship with the various stakeholders and participants in the community to
address the issue of substance abuse hence will follow the following strategies.
1. Family centered solution – This strategy is intended to empower parents on the
prevention and treatment of substance abuse. New Life Charity Organization believes the
family is the most basic structure in the community that can address risk factors for
substance abuse while insuring success in the treatment of substance abuse. Therefore,
the organization will work closely with families in the prevention and treatment of
substance abuse.
2. School-Based Substance Abuse Prevention – This strategy is aimed at learning best
practices from schools on how to combat substance abuse, transferring best practices
among schools and empowering schools to take leading role in the prevention of
substance abuse.

3. Empowering mothers – Women have a powerful and unique role in families all across
Ethiopia. As a result this strategy seeks to develop the parenting skills of mothers, to help
them detect early behavioral changes in their children while arranging the transfer of
knowledge from mother to mother.
4. Recovered victims of substance abuse as New Life ambassadors – This strategy uses
recovered victims of substance abuse within the community to show other victims that
recovery is possible. New Life ambassadors will also share their past experience to the
community in order to prevent others from becoming victims of substance abuse.

X. Project Risks, Monitoring & Evaluation


Risks to successful implementation New Life Charity organization assumes the following
risk to the successful implementation of the project.

 Reduced cooperation and lack of concerted effort by all stakeholders toward a common
goal of reducing the harmful effect of substance abuse.
 Capacity constraints in terms of facility, equipment and other resources to cope up with
rising demand for treatment and prevention
 Not enough funding for the project – Although the organization plans to pursue different
strategies in order to insure self sustainability in the long term, in the short term the
organization may face some constraint arising from not enough funding.

Monitoring, Evaluation Plan and Indicators


Monitoring
New Life Charity organization will periodically monitor its program through systematic and
routine collection of information in order to foster internal and external accountability of
resources used and results obtained. Monitoring will also enhance the organization’s capability
by providing lesson to improve practices, guiding decision making, and empowering
beneficiaries. The information obtained through monitoring will be communicated to signatory
government offices, partner organizations and funding entities through a quarterly report.

Evaluation
The organization will conduct annual and terminal evaluations with the collaboration of
signatory government offices under the leadership of the Ethiopian Ministry of health to assess
the relevance, effectiveness, efficiency, impact, and sustainability of the project.

Monitoring and Evaluation Reports


The project coordinator will be responsible for monitoring the day-to-day activities of the project
and furnishing periodic activity and financial report to the executive director. In turn; the
executive director with the board of directors will revise the reports and provide it to the Ministry
of Health and other signatory partners, periodically as per the agreed timeframe and format.
