Functional Health Pattern Before Hospitalization During Hospitalization

Nutritional pattern Patient eats three meals a day. He usually eats food that Patient was put on NPO.

brandy during night time to make him sleep and

nuts as their “pulutan”.
Elimination pattern Patient regularly urinates around 8 times in a day. He Patient was on foley catheter that has a urine output of
usually defecate during daytime before he goes to work 50 cc per hour.

the morning
Sleep & Rest pattern Patient usually sleeps around 10pm but if he does not Patient is having hard time to sleep because of the
drink alcohol he usually sleeps around 11pm – 12 environment inside the ward and he usually wakes up in

the ward.
 Activity and Exercise Patient’s daily activity is to go to work and during his day
off he usually stays at home talk to some of his
neighbours and watch DVD. He also smoke cigarette and
consumes half a pack to one pack of cigarettes a day.
Cognitive Perceptual Pattern Patient has a normal hearing, tactile, olfaction and
gestation. His vision is slightly blurred and uses reading
glasses whenever he reads usually the newspaper
Self Perception Pattern Patient said that he gets easily mad when people around
him commit mistakes especially if the work to be done is
easy. He also said that he is afraid of injection, when he
sees blood.
Role Relationship Pattern Patient does not have any bad issues on his wife.
Whenever the child of his wife commit mistakes, he
reprimand the child instead of spanking it or doing any
physical harm.
Sexuality Pattern Patient is having a sexual intercourse at least 2 times in a
week. He do not use condom or any contraceptive
measures. That patient also said that he do not feel the
pain in his left lower quadrant because he lies and his
wife is the one doing the work.
Coping Stress Tolerance Patient stated that whenever there is a problem inside
their house which it usually seldom to happen, he goes to
his siblings and ask for help of assistance
Values belief pattern Patient is a Methodist. He usually go to church on a
regular basis.
Patient usual activity inside the ward is to talk to his
partner and to other patient’s significant others. He also
sleep if he does not having a communication. He admits
that he wants to smoke badly.
Patients does not have any cognitive or perceptual
disturbances besides that his vision remain to be slightly
blurred.
Patient is nervous on the operation that is to be done on
him. He also became patient when he was hospitalized.
Patient said that his wife who is hands on in taking care
of him when he was hospitalized.
Patient states that he wants to have sex but he cannot
because they are in the hospital. He also stated that his
condition does not affect his manhood.
Patient is now anxious to his condition especially the
procedure that he is about to be done on him. To lessen
his anxiety he just talks to his wife.
Patient cannot attend church because of
hospitalization. And also he prays that his condition will
get better.
