Objective
The FASTag instrument comes under PPI (Prepaid Payment Instrument) category and it is obligatory
on the Bank as an issuer to comply the Master Guidelines of RBI on KYC and to keep updated records
for verification by the inspecting officials of RBI or any other government agency. In terms of which,
obtaining, verification, and safekeeping of the KYC records along with the FASTag application are the
primary responsibilities of Agents/VLE/Dealer as per the KYC directions of RBI. To avoid any non-
compliance and resulted penalty from RBI for non-compliance of KYC guidelines, please follow to
expedite the collection and arrangement of the KYC credentials of the FASTag customers as detailed
hereunder:
1- FASTag application form for all FASTag customers should be duly filled with required
information of the customer and their respective vehicle(s), the application form and Xerox
copies of KYC documents should be signed by the respective customers and
Agents/VLE/Dealer also.
2- The FASTag application form and KYC documents of the customers should be duly verified by
the Agents/VLE/Dealer etc. with their official stamp and full signatures
3- The KYC documents should have certification of “Verified with original” with the official
stamp of the Agents/VLE/Dealer.

Procedure:-
FASTag is a semi closed Pre- Paid Instrument (PPI) for which the Bank is following KYC guidelines
revised from time to time by RBI through their Master Directions – Know Your Customer (KYC)
Direction, 2016.
During customer registration, the customer is registered as one of the two below mentioned entities:
1. Individual
2. Corporate
For individual entities, the application has option to register the customer under Minimum Details or
Full KYC. On the other hand, for Corporate Customers, option is available for registration under Full
KYC only.
The Agents/VLE/Dealer appointed by the CSC SPV perform registration of the customers. The following
steps will be followed by the Agents/VLE/Dealer during registration:
1. The customer fills in the application form as per the type of account required
2. The Agents/VLE/Dealer registers the customer through the portal designed specifically for
issuance of FASTag
3. For Minimum Details (Limited) KYC customers, the Agents/VLE/Dealer needs to take the
number for one of OVDs on self-declaration basis as directed by RBI, apart from Name and
Mobile Number. The OVDs as mentioned as below:
a. The passport
b. The driving License
c. The Voter's Identity Card issued by Election Commission of India,
d. Job card issued by NREGA duly signed by an officer of the State Government,
e. The letter issued by the National Population Register containing details of name, address,
or any other document as notified by the Central Government in consultation with the
Regulator.
4. For Full KYC Customers: The required documents i.e. ID proof and Address proof as mandated
by the RBI guidelines for issuance of PPIs are taken by the Agents/VLE/Dealer. The KYC
document obtention, verification and preservation should be as per RBI guidelines from time
to time. The KYC documents for different customer categories with effect from 26th
September 2018 are given below:
i. The passport
ii. The driving License
iii. The Voter's Identity Card issued by Election Commission of India,
iv. Job card issued by NREGA duly signed by an officer of the State Government,
v. The letter issued by the National Population Register containing details of name, address, or
any other document as notified by the Central Government in consultation with the Regulator.

B) Where the customer is a Proprietorship Firm:
i) Minimum two documents issued in the name of Proprietary Concern from the following list of
documents along with any OVD and PAN of the Proprietor as a beneficial owner must be taken.
ii) Proof of the name, address and activity of the concern like registration certificate (in case of
a registered concern)
iii) Certificate/ License issued by Municipal Authorities under Shop and Establishment Act.
iv) Sales and Income Tax returns
v) GST/CST certificate, Certificate / registration document issued by sales tax / Service tax/
Professional tax authorities.
vi) License / Certificate of Practice issued in the name of proprietary concern by any professional
body incorporated under statue (e g Certificate of Practice issued by Institute of Chartered
Accountants of India, Institute of Cost Accountants of India, Institute of Company Secretaries
of India, etc)
vii) IEC (Import / Export code) issued to the Proprietary concern by the Office of Director General
of Foreign Trade (DGFT) in the name of Proprietary Concern.
viii) The complete Income tax return (not just acknowledgement) in the name of sole proprietor
where the Firm’s Income is reflected duly authenticated/acknowledged by the Income tax
authorities.
ix) Utility bills such as electricity, water and landline telephone bills in the name of the proprietary
concern.
i. Certificate of incorporation
ii. Memorandum and Articles of Association
iii. A resolution from the Board of Directors and power of attorney granted to its managers,
officers or employees to transact on its behalf
iv. PAN of the Company
Other documents
v. A declaration containing the names of all the beneficial owners together with their
shareholding / controlling interest / stake duly signed by the authorized signatory.
vi. Certificate of commencement of business (in case of Public Limited Company)
vii. CIN No. ……………………..
viii. Copy of PAN of Company
ix. Proof of Current Address
x. Any officially valid document/ Identification of those who have authority as per POA granted
to operate the account (as applicable to individual accounts) and KYC of all such persons
operating the account and beneficial owners

xi. Certificate of Registrar of Joint Stock Companies dated ……………………That the Company is
entitled to commence business (for inspection, entry in the Power of Attorney Register and
return). A copy of the same is retained. (This certificated is not required when, a. The
company is a private company b. The company was registered before 1913 and does not
invite the public to subscribe for shares. c. The company is limited by guarantee and does
not have a share capital).
xii. Certified copy of a resolution dated ……………………, regulating the conduct of the account,
obtained, somewhat on the following lines: We hereby certify that the following resolution
of the Board of Directors of the ……………………Company was passed of a meeting of the Board
held on the ………………And has been duly recorded in the Minute Book of the said Company :-
“resolved :- that a bank account for the company be opened with the Bank, and that the said
Bank be and is hereby authorised to honour cheques, bills of exchange and promissory noted
drawn, accepted or made on behalf of the company by ……………………and to act on any
instructions so given relating to the account, whether the same be overdrawn or not, or
relating to the transactions of the company. (Should be signed by chairman, directors and
secretary of the company jointly)
xiii. Officially valid documents as defined by RBI from time to time and PAN of the Chairman /
Managing Director / Chief Promoter etc. of all related persons or beneficial owners.
Other documents
v. A declaration containing the names of all the beneficial owners together with their
shareholding / controlling interest / stake duly signed by the authorized signatory.
vi. Mode of operation in case of Partnership to be indicated clearly in AOF (Viz. All Partners
jointly/severally (singly), Partner 1 & 2 jointly/severally (singly) etc.)
vii. Partnership letter dated……………and No. ……………obtained on Cos 37. Signed by all partners.
(To be compulsorily obtained in case of partnership firms)
viii. Addresses of the Power of Attorney holders
ix. PoA granted to a partner or employee of the firm to transact business on its behalf
x. OVD as defined by RBI from time to time and PAN of all partners & beneficial owners

E) Where the customer is Societies/Association/Clubs:
i. Joint Hindu Family Letter dated ………………………And No. …………Obtained on Cos 38, signed by
all the adult coparceners Family (HUF)
ii. Declaration from the Karta
iii. OVD as defined by RBI from time to time and PAN of Karta
iv. OVD as defined by RBI from time to time and PAN of adult coparceners
v. PAN Card of Joint Hindu Family
vi. On death of a coparcener, birth of a coparcener and a minor coparcener attaining majority (18
years), a fresh JHF letter (COS 38) has to be executed Declaration that a) the depositor is the
Karta of the Joint Family, b) the deposit belongs to JHF
Other Documents
v. A declaration containing the names of all the beneficial owners together with their
shareholding / controlling interest / stake duly signed by the authorized signatory.
vi. Copy of relevant extracts of trust deed dated ……………obtained and perusued, with special
emphasis on the power of the trustees to sign cheques, delegation of authority, borrow
money etc. The relevant portions are entered in the power of attorney register.
vii. A copy of the Resolution
viii. Power of Attorney granted to transact business on its behalf (wherever applicable),
ix. OVD as defined by RBI from time to time and PAN of trustees, executors, administrators,
etc. of all related persons or beneficial owners.
x. Proof of current address
xi. All Trust Accounts to be invariably assigned “High Risk”

At any point of time, Agents/VLE/Dealer will ensure the below
given points as per RBI and Bank guidelines from time to time:
Duly verified scanned copies of these documents will be uploaded by the Agents/VLE/Dealer
in the portal provided to them
These documents are collected by the Agents/VLE/Dealer and sent to the CSC SPV central
address.

After the Agents/VLE/Dealer has completed the registration for the customer, the form and
the soft copy of the uploaded documents will be available in the portal for verification.
Going ahead as per the revised guidelines of the Bank for pre-paid instruments (PPIs), it is
proposed to make PAN mandatory (in addition to the mobile number) while registration to
the customer. Mobile along with the PAN number would be unique identifier.
