注意: (1) 通知書須由結婚雙方的其中一方或雙方填寫及簽署，並由當地認可的公証人認證。

(2) 遞交通知書時，須連同結婚雙方的香港身份證或旅行證件副本一併寄回。
(3) 持有有效香港身份證的申請人必須以香港身份證上的資料遞交申請。
(4) 擬結婚通知書存檔及展示的費用為港幣三百零五元。
(5) 擬結婚通知書上所填寫的姓名及出生日期必須與香港身份證或旅行證件上的資料完全相符。

Notes: (1) This form should be completed and signed by one or both of the marrying parties giving the notice and authenticated by a
Notary Public.
(2) Photocopies of Hong Kong Identity Cards or travel documents of the marrying parties are required for giving
this notice.
(3) Applicants holding valid Hong Kong Identity Cards should submit the application with the particulars shown
on the Hong Kong Identity Cards.
(4) Fee for filing of this notice is HK$305.
(5) The Notice of Intended Marriage has to be prepared with the particulars of both of marrying parties matching
exactly with that on the Hong Kong Identity Card or the travel documents.

說明事項 Supplementary Notes
更正錯誤 Correction of Error
根據《婚姻條例》（第 181 章）第 25 條的規定，婚姻登記官只可更正已交給結婚雙方的結婚證書上的文書錯誤。任何於該等結婚證書上的事實錯誤或實質錯誤將無法更改，該
條例亦無賦予婚姻登記官重發結婚證書的權力。
Under section 25 of the Marriage Ordinance (Chapter 181), only clerical errors on marriage certificate already delivered to the parties may be corrected by the Registrar of Marriages. There
will be NO correction of error of fact or substance after the marriage ceremony. This Ordinance has not empowered the Registrar of Marriages to issue a replacement marriage certificate.

收集資料的目的 Purpose of Collection

資料表內所提供的個人資料會供入境事務處作下列一項或多項用途：－
The personal data provided in this information sheet will be used by Immigration Department for one or more of the following purposes:-
1 辦理婚姻登記、翻查及簽發婚姻紀錄及有關申請；
to process the marriage registration, search and issue of marriage records or related applications;
2 執行有關婚姻登記的法例，包括在舉行婚禮前把擬結婚通知書在公眾地方展示；如任何人士根據法例有權利反對這段婚姻，可以書面向婚姻登記官提
出；
to administer the marriage registration related ordinances including the requirement to exhibit the Notice of Intended Marriage in places open to the public before
the marriage ceremony takes place; any person entitled at law to object to the proposed marriage may do so by notice in writing addressed to the Registrar of
Marriages;
3 實施／執行《入境條例》（第 115 章）及《入境事務隊條例》（第 331 章）的有關條文規定，以及履行入境管制職務，藉此協助其他政府決策局和部
門執行其他法例和規例；
to administer/enforce relevant provisions of the Immigration Ordinance (Chapter 115) and Immigration Service Ordinance (Chapter 331), and to assist in the
enforcement of any other Ordinances and Regulations by other government bureaux and departments through carrying out immigration control duties;
4 在有關人士向入境事務處提出申請並提名你為保證人或諮詢人時，把你的資料供作核對用途；
To process other person’s application for immigration facilities in which you are named as a sponsor or referee;
5 供作統計及研究用途，但所得的統計數字或研究成果，不會以能辨識各有關的資料當事人或其中任何人的身份的形式提供；以及
for statistics and research purposes on the condition that the resulting statistics or results of the research will not be made available in a form which will identify
the data subjects or any of them; and

The provision of personal data in page 1 to page 3 of this information sheet is obligatory to fulfill statutory requirements prescribed by the Marriage Ordinance
(Chapter 181). Such data will be used to compile Notice of Intended Marriage, Certificate of Registrar of Marriages and Marriage Certificate, to fulfill and administer
the Marriage Ordinance and other related purposes, including the issue of the Certificate of Marriage, the consent to searches to be made amongst all certificates,
licences, registers and indexes in the possession of the Registrar of Marriages from the members of public and the provision of the person conducting the search with a
certified copy of the relevant record. If you do not provide sufficient information, Immigration Department would not be able to process your marriage registration.

The personal data you provide may be disclosed to government bureaux, departments and other organisations for the purposes mentioned above.

You have the right of access including the right to obtain, upon payment of the prescribed fee, a copy of your personal data provided by this information sheet.
2 如你未能在預定日期前往指定婚姻登記處遞交擬結婚通知書，你經本處網上預約系統提供的所有個人資料，均會在預定日期後立即清除。有關的個人
資料被清除後，你不能根據《個人資料（私隠）條例》（第 486 章）要求查閱或更正該等資料。
If you fail to show up for giving the Notice of Intended Marriage at the designated Marriage Registry on the appointed day, all the personal data provided by you
via our on-line booking system will be purged immediately after the appointed day. There can be no access to or correction of those data under the Personal Data
(Privacy) Ordinance (Chapter 486) after the purging of the personal data.
3 婚姻登記過程中所收集的個人資料包括：結婚日期、籍貫、教育程度、曾締結婚姻次數、前結緍日期、離婚日期/前配偶死亡日期、個人婚姻狀況、
職業、年齡、居住香港時間、前居住國家及所操語言，乃是供政府統計處製備統計數字及進行研究之用。所得的統計數字及研究結果將不會識辨當事
人或其中任何人的身分。因此，並未能根據《個人資料（私隱）條例》（第 486 章）而要求查閱或更正該等資料。
The following personal data are also collected during the marriage registration on behalf of the Census & Statistics Department for statistics, analysis and research
purposes: date of marriage, place of origin, education attainment, number of previous marriage, data of previous marriage, divorce date/last spouse’s death, marital
status, occupation, age, length of stay in Hong Kong, country of previous residence and language. The resulting statistics and results of the analysis & research are
not made available in a form which identifies the data subjects or any of them. There is no subsequent access or correction of these data under the Personal Data
(Privacy) Ordinance (Chapter 486).
