TRAINING AGREEMENT AND NON-LIABILITY WAIVER

I, LM - Integrated System and Data Networking, Inc., agrees to permit Astroval R. Guita , BS in
Electronics and Communication Engineering of Cagayan State University, College of Engineering to
work on this establishment under the On-the-Job Training Program in the phase of his/her field of study.
It is agreed that:

1. The duration of the OJT program is minimum of 240 hours as an academic requirement for the
graduation and the training program starts on ________________and ends on ______________
or upon the completion of the 240 hours;
2. The Student agrees, while on the process of training will have the status of student trainee,
neither displacing a regular worker presently employed nor substituting the worker needed by the
Cooperating Agency.
3. The Student agrees to perform diligently the work assigned to him/her by the Cooperating
Agency. He/she also agrees to pursue faithfully the prescribed course of the study and to take
advantage of the opportunity to improve his/her efficiency, knowledge and personal traits so that
he/she may enter his/her choice of occupation as a desirable worker after graduation.
4. The Coordinator/Faculty In-charge shall have the authority to transfer or withdraw the student at
any time a situation warrants such as change of assignments;
5. The Cooperating Agency shall evaluate the rate of student’s performance and discuss with
him/her pertinent issues regarding the training program;
6. All complaints regarding the conducts and performance of the student shall be forwarded to the
Coordinator/Faculty-in-Charge;
7. The students shall abide to all Cooperating Agency’s rules and regulations and shall comply with
those imposed for the training program. Otherwise, he/she shall be excluded from further
participation.
8. There is No Labor-Management relationship between the Student and the Cooperating Agency;
9. The Student renounces and waives any claim against the Cooperating Agency for any injury
he/she may sustain or loss he/she may suffer, personal or pecuniary, in the performance of
his/her duties and functions while on-duty, provided that the Cooperating Agency exerts utmost
effort for the safety and welfare of the Student while on duty; and
10. The Students shall be occasioned by his/her intention or negligence while on-duty, provided that
the Cooperating Agency exercises diligence in the supervisions/instructions to the student while
on duty.
