The patient reported pain at the abdominal incision site from surgery. On assessment, the patient displayed facial grimacing and reported pain at a level of 5/10. The nursing diagnosis was acute pain related to inflammation from the surgical incision and appendectomy. Goals were for the patient to report reduced pain within 4 hours and appear more relaxed. Interventions included pain medication, rest in semi-Fowler's position, early ambulation, distraction, and ice for pain relief. Evaluation found pain reduced to 3/10 and the patient appeared relaxed after 4 hours.
