AFFIDAVIT OF DISCREPANCY

I, WENDELL M. VILLACARLOS, of legal age, Filipino, married, and

a resident of Banilad, Mandaue City, Philippines, having been duly sworn
in accordance with law, hereby depose and state:

1. That I am the same WENDELL M. VILLACARLOS who is the

affiant herein, a copy of my Birth Certificate is herewith attached
as Annex “A”.

2. That I have always used the name “WENDELL M.

VILLACARLOS”, such that in all my other documents, including
valid identification cards, my name was reflected as “WENDELL
M. VILLACARLOS”;

3. That my owner’s duplicate copy of Transfer Certificate of Title

No. MR-10653 (T-63866) was lost, and upon issuance of a new
owner’s duplicate copy, the name recorded as the owner of the
property was “WENDEL” instead of “WENDELL”.

4. That the names WENDEL M. VILLACARLOS and WENDELL M.

VILLACARLOS refer to one and the same person;

5. Further, I am declaring that my true and correct name is

WENDELL M. VILLACARLOS;

6. That I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose this may serve.
