resident of Barrio Cavit (43), Laoag City, Ilocos Norte, Philippines, under oath, do hereby
depose and state that:

The said motorcycle was confiscated and impounded by PNP-Laoag City in relation
to a vehicular traffic accident which was duly recorded in the Official Police Blotted of the
Laoag City Traffic Division under Entry No. 0007169;

I am executing this affidavit to support my request for the release of the impounded
motor vehicle;

Anent thereto, I hereby undertake to produce said vehicle upon request of the proper
authorities if the same would be needed for further proceedings or investigations;

Moreover, I am executing this affidavit to attest to the veracity of the foregoing

facts for all legal purposes it may serve;
